The Dayton, Ohio Veterans Affairs Medical Center's Pharmacy Service is recruiting for a well-qualified Clinical Pharmacy Specialist (Pharmacogenomics Clinical Pharmacy Practitioner Provider). The Clinical Pharmacy Specialist functions at the highest level of clinical practice and top of licensure and serves as an advanced practice provider working independently, but collaboratively, with prescribing privileges as outlined in the individual's practice area-based scope of practice.
Duties
FUNCTIONS OF THE POSITION:
a. Clinical - Direct Patient Care Activities:
Functions as advanced practice provider for CMM services to achieve defined outcomes through direct interactions with patients and providers
Functions as part of the interdisciplinary team to provide PGx-based CMM
Performs limited external physical assessment, performs medication reconciliation
Provides CMM for complex patients with chronic diseases; orders and interprets labs or other needed tests
Determines if a referral to physician is necessary when disease progression occurs, or adverse drug events require higher level of treatment intervention
Accepts urgent or same day patient visits, recent hospital or emergency department discharges, co-managed care patients.
b. Clinical - Indirect Patient Care Activities
Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, therapeutic outcomes.
Facilitate the creation of pharmacist-led specialty PGx services
Monitors and reports drug errors, adverse drug reactions, allergies, patient compliance issues.
Reviews relevant population management dashboards to identify patients who may benefit from engagement in care for PGx testing, management, and/or risk mitigation
Assists Pharmacy and Therapeutics (P&T) committee with pharmacogenomics reference laboratory and PGx test evaluations, other safety initiatives
Apply principles of comprehensive medication management to clinical PGx program
c. Program Management, Educational, and Administration:
Advocate for rational and ethical use of PGx testing as part of routine care
Lead diffusion of PGx pharmacist train the trainer program to offer training to pharmacy leaders, clinical pharmacists, clinical pharmacy practitioners, clinical pharmacy Practitioners, pharmacy residents
Facilitate PGx test results are handled in ethical manner and that patients are provided access to their genetic data when applicable
Provides formal and informal education and in-services to pharmacy, medical, nursing, other staff as requested.
Serves as subject matter expert in PGx for assigned committees, workgroups, interdisciplinary teams.
Provides education to patients, their families and caregivers about medications, patient responsibilities, information on health promotion.
Participates in self-development activities to maintain and strengthen professional competence in PGx and precision medicine
Willing to serve as clinical instructor or preceptor for pharmacy students and/or residents.
Assists in new employee orientation education sessions as needed.
Participates in appropriate, effective, safe use of pharmaceuticals by contributing to criteria development, data collection, quality assurance and drug use evaluation, other quality improvement activities for core practice area(s) medication use and PGx.
Assists in facilitation, evaluation, implementation of existing, nationally available, PGx programs (e.g., the pharmacogenomics testing for Veterans (PHASER) program) as integral part of medication therapy if the program is not available locally
Assists in facilitation, evaluation, implementation, in collaboration with local laboratory services, to offer PGx testing where it is not available
Create clinical workflows to facilitate PGx testing for specific patient cohorts
Consider creation of drug-gene workflow to facilitate PGx testing
Administratively manage PGx pharmacist train the trainer program tracking activity and reporting these data to PGx National
Serves as pharmacist subject matter expert on assigned PGx related committees and subcommittees.
Supports and promotes compliance with established drug therapy policies, formulary management and criteria or prior authorization requirements that have been approved by national, VISN, or local pharmacy benefits management (PBM) and focuses on quality patient care in cost-effective manner.
Participates and facilitates departmental and organizational compliance with applicable regulatory and accreditation regulations and guidelines
Supports efforts to improve performance metrics specific to PGx and medication use.
Participates in committee functions and assignments as required.
Maintains effective interdepartmental relationships by committee membership, barrier resolution, active problem resolution skills, and close cooperation with other services in accomplishing medical center mission and goals.
d. Research: (when applicable)
Participates in design and/or completion of research studies and/or quality improvement projects involving drug therapy, clinical pharmacy functions, or improvements in patient care.
Directs and/or participates in preparation of articles for publication.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
Citizenship
Citizen of the United States.
Education
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.
(1) Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed.
(2) Failure to Obtain License. In all cases, pharmacists must actively pursue meeting state prerequisites for licensure starting from the date of their appointment. At the time of appointment, the supervisor will provide the unlicensed pharmacist with the written requirement to obtain licensure, the date by which the license must be acquired, and the consequences for not becoming licensed by the deadline. Failure to become licensed within 2 years from date of appointment will result in removal from the GS-0660 Pharmacist series and may result in termination of employment.
Grandfathering Provision
All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
(1) Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
(2) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
(3) If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
English Language Proficiency
Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Grade Requirements
Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
1. 1 year of experience equivalent to the GS-11 grade level, or 2. Completion of an ACPE-accredited Pharm.D. program.
They must also have 1 year of experience equivalent to the GS-12 grade level.
Pharmacists assigned to this position must demonstrate the following knowledge, skills, and abilities to complete the duties of the position, including the necessary skills to provide care appropriate to the population of the patients served.
i. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
ii. Expert understanding of regulatory and quality standards for their program area.
iii. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
iv. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
v. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment, and interpretation of laboratory, advanced diagnostics, and other diagnostic parameters.

The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is GS-13.
