The Community Care Division Nurse Case Manager is responsible for the independent assessment, planning, implementation, and evaluation of clinical case management activities necessary to purchase patient care in the community, in a manner that successfully integrates the community care with past and future VA care

Duties

The Community Care Nurse Case Manager's duties and responsibilities include but are not limited to the following: Assessment, planning, and implementation of clinical plan of care for patients hospitalized in non-VA facilities Assessment and identification of patients appropriate for community care services Review and clinical triage of community care consults for simplistic and complex care Coordination of complex clinical care for the patient with the VA referring provider and the community provider Collaboration with the authorization staff to ensure clinical care is progressing, efficient, while maintaining fiscal responsibility Evaluating Veterans appropriate for re-integration back into the VA system, when community care episode is completed Work collaboratively with all clinical divisions, pharmacy, prosthetics, travel/lodging, etc. across the VA Portland Health Care System (VAPORHCS), other VISN 20 sites, and community providers to facilitate appropriate care Work collaboratively with the administrative staff within the Community Care Division to ensure coordinated, timely, cost-efficient care to the Veteran. Demonstrate leadership, experience, and creative approaches to the management of complex patient care. Frequently act on behalf of VAPORHCS with other VISN20 sites to determine availability of clinical services, ensure proper transfer of patient care, participate in collaborative case management planning, utilization review and other related case management activities. Based on education and experience, the Nurse Case Manager may be assigned responsibility as preceptor and/or charge nurse. Intermittent, Temporary positions (1 year or less) that do not work 130 hours per month for 90 days may not be eligible to earn paid time off (leave) and may not eligible for the Health Benefits (Insurance) or Retirement. Work Schedule: Monday - Friday; 7:30am - 4:00pm.Position is intermittent and actual hours will be determined by service needs. Telework: Available; This position is 100% virtual but you must be within 50 miles of the facility. Virtual: This is a virtual position. Financial Disclosure Report: Not required

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Preferred Experience: BSN, care coordination experience, critical thinking, and problem solving skills are all preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - Diploma or associate degree in nursing with no professional nursing experience Nurse I Level II - Diploma or associate degree in nursing and one year of specialized nursing experience equivalent to Nurse I, Level 1 OR a Bachelor's degree in nursing and no professional nursing experience. Nurse I Level III -Diploma or associate degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 2 OR Bachelor's degree in nursing and one year of professional nursing experience equivalent to Nurse I, Level 2 OR Master's degree in nursing and no professional nursing experience OR Master's degree in a related field with a bachelor's degree in nursing with no professional nursing experience. Nurse II - Bachelor's degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 3 OR Master's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3 OR Master's degree in a related field with a bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3 OR Doctorate degree in nursing with no professional nursing experience OR Doctorate degree in a related field with a master's or bachelor's degree in nursing with no professional nursing experience. Nurse III - Master's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III OR Master's degree in a related field with a bachelor's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III OR Doctorate degree in nursing with one year of specialized nursing experience which is equivalent to Nurse II and meets all dimension requirements for Nurse III OR Doctorate degree in a related field with a master's or bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Light lifting, under 15 pounds; Light carrying, under 15 pounds; Use of fingers; Both hands required; Hearing (aid may be permitted). Environmental Factors: Working alone.

(1) Education Accreditation Requirement: Nursing education must be from a school of professional nursing accredited by one of the following accrediting organizations at the time the program was completed by the applicant:
(a) The Accreditation Commission for Education in Nursing (ACEN), 3343 Peachtree Road NE, Suite 850, Atlanta, GA 30326. The ACEN accredits all levels of nursing programs, including clinical doctorate, masters/post master's certificate, baccalaureate, associate and diploma nursing education programs. (Note: The National League for Nursing Accrediting Commission/NLNAC changed its name to ACEN in May 2013. ACEN's website may be utilized to verify accredited programs using automated inquiry processes; however, for verification of any nursing program's accreditation prior to 1998, an automated inquiry cannot be used and ACEN must be contacted directly via their email contact information on the ACEN website to verify.); or
(b) The Commission on Collegiate Nursing Education (CCNE), 655 K Street, NW, Suite 7, Washington, DC 20001. The CCNE is an accrediting arm of the American Association of Colleges of Nursing (AACN) and accredits nursing education programs at the bachelor and master's level. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

(2) Foreign Education: Graduates of foreign schools of professional nursing who possess a current, full, active and unrestricted registration (see paragraph 3c) will meet the requirement of graduation from an approved school of professional nursing to warrant appointment as a Nurse who has completed an associate degree level Nursing education program. In order to grant credit towards an appointment based on foreign nursing education higher than the associate degree level, formal degree equivalency validation from a recognized degree equivalency evaluation agency accepted by VA such as the International Consultants of Delaware (ICD) must be obtained.
