The incumbent is primarily responsible for patient and clinic flow, efficiency of the clinic, customer service, and teamwork. The LPN develops/maintains effective and efficient communication with the patient, interdisciplinary team, VA medical centers, and other agencies. Other assignments at this level include, but are not limited to: checking vitals, processing reminders, reviewing and processing alerts appropriately, administering medication, participating in team huddles and meetings to manage and plan patient care; setting priorities and deadlines; adjusting the flow and sequencing of the work to meet team and patient needs; may work with the team to reinforce the plan of care and self-help solutions and entering appropriate information into the electronic record; monitoring pre-appointment requirements to assure readiness for patient visit/procedure (e.g., X-ray, lab work); participating and providing input in problem solving on operational issues or procedures in team meetings; participating in and independently following up on team huddles by sharing information and collaborating with the medical team to assure continuity of care; evaluating patient information and clinic schedule lists to determine whether patient is vested; educating providers about shared patients (those who receive their care at multiple VAs or those who have care in the community) and bringing to the attention of the provider. At this level the LPN is high functioning and work towards the top of their scope. Duties listed below are at the GS-6 level, developmental grades will differ in complexity and level of supervision. In this position the LPN is more than likely assigned to a Patient Aligned Care Team (PACT) and is responsible for assisting with panel management for his/her PACT, and other duties as assigned by supervisor or per the daily schedule. Qualifications

To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement. Basic Requirements: United States Citizenship: Citizen of the United States. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the Accreditation Commission for Education in Nursing (ACEN) at the time the program was completed by the applicant. Exceptions include: Health care education in the military service or training in the military service that is accepted by the licensing body in the jurisdiction in which licensed as qualifying for full LPN/LVN licensure meets the education requirements. Individuals granted a license by a jurisdiction that does not require graduation from an approved school. Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN/LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.
