The Pharmacist is responsible for the daily professional, administrative operations and for effective utilization of resources and coordination of the Pharmacy Community Care Services at VA Northern California Healthcare System.

Under the direction of the Outpatient Pharmacy Supervisor and in counsel with Pharmacy Leadership, the Pharmacist has full responsibility for the daily management and operation of the assigned section according to Pharmacy Service philosophies, policies, procedures, goals, and objectives. The Pharmacist will assist Pharmacy leadership in ensuring the Service meets all oversight standards and functions, as well as service-related performance measures and monitors. Duties include but are not limited to the following:
Clinical
Uses high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the workday.
Performs all duties of a clinical pharmacist in the outpatient setting.
Demonstrates efficiency by working with the outpatient supervisor setting/achieving performance goal standards.
Ensures patient medication regimen is assessed at every encounter.
Ensures adverse drug events, clinical interventions, patient drug therapy evaluations and counseling are documented in electronic medical record.
Establishes and monitors pertinent medication management system and performance improvement indicators to assure quality services are provided.
Ensures appropriate drug information is provided to patients, physicians, nurses, pharmacy staff, and other health care professionals.
Education and Training
Identifies educational priorities and makes provisions for the development and training needs of subordinates.
Develops and performs in-services promoting cost-effective medication management to all within the Healthcare System.
Participates in the precepting of pharmacy residents and students.
Provides and coordinates job trainings and information to staff.
Maintains general knowledge of VA, DEA, OIG, FDA, and The Joint Commission regulations.
Tracks, trends, and evaluates assigned metrics.
Administrative
Ensures implementation of pharmacy-based actions.
Provides written reports and/or correspondence to health care system management.
Ensures accuracy, validity, and timely submission of pharmacy workload, systematic review/performance improvement, operations, drug utilization and reports.
Maintains knowledge, skills, and flexibility to be assigned interchangeably in all areas of Pharmacy Service.
Ensures protection of printed and electronic files containing sensitive information from unauthorized disclosure, loss, unauthorized alteration, or unauthorized deletion.
Assists Pharmacy Leadership in efficiently managing the facility's medication budget.
Develops and implements appropriate procedures, maintains compliance with local, VISN, national VA requirements, policies, and procedures as well as DEA, OIG, FDA, and TJC standards.
Develops and maintains a cooperative working relationship with other departments.
Assist in the development of long and short-term goals for the service and implements short-range goals.
Program Management
Monitors, plans, and organizes daily operation of Pharmacy Community Care services.
Ensures services provided meet or exceed professional and legal standards.
Effectively organizes and controls Pharmacy Community Care related activities, plans short- and long-range objectives, solves problems, and achieves results within budgetary and staffing constraints.
Collects, analyzes, and acts upon customer service, workload, quality assurance, productivity, and workflow reports.
Maintains continuity of care to ensure the provision of prompt and accurate pharmaceutical services.
Identifies new performance measures to improve quality of care.
Work Schedule: Variable shift - rotation during the hours of operation.

Paid Time Off: 37 days of annual paid time offer per year (13 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

BASIC REQUIREMENTS
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education.
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy).
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed. Please note that this position is at the GS-12 level only.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
Physical Requirements. TB/Immunization Assessment and a pre-employment physical exam will be conducted by Employee Occupational Health Service.
GRADE DETERMINATIONS
In addition to the basic requirements for employment stated above, the following criteria must be met when determining the grade of candidates.
Preferred Experience: Minimum two years of experience.
GS-12 Outpatient Pharmacist
Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
One year of experience equivalent to the next lower grade level, or
Completion of an ACPE-accredited Pharm.D. program.
Assignments. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
Knowledge of professional pharmacy practice.
Ability to communicate orally and in writing to both patients and health care staff.
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

The full performance level of this vacancy is GS-12. Selection will be made at the GS-12 level.
Significant Information regarding Physical Requirements: The position requires visual and auditory acuity at a level to provide safe and effective care. Use and function of all normal motor and sensory anatomy for provision of patient care, reading, writing, listening, observing/seeing, heavy lifting, bending, pulling, pushing, stooping, squatting, walking, standing, use of fine motor muscles. Must be able to sit for 3 hours, stand for 3 hours, and walk for 1 hour. The position also requires ability to work on computers for extended periods of time. Requires use of hands for actions such as typing, simple grasping, fine manipulation, and repetitive actions.
Requires the ability to frequently respond to multiple demands and priorities; emotional intelligence to respond to stressful and/or emergent situations and crises appropriately and calmly; self-care skills to maintain physical, psychological, and emotional health. Heavy lifting of 35 pounds and over, heavy carrying of 35 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously. Has potential for exposure to infectious diseases, hazardous waste and chemicals.
