Duties

Rehab and Extended Care (REC) RN REC RN's are responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. REC RN's will assesses, plan, implement, and evaluate all care based on age-specific components. REC RN's assume responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge and supporting factors that impact customer satisfaction. REC RN's will work with the care team to sign off and ensure completion of all orders written for their assignment and that of their LPN teammates. REC RN's will consider all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. During a shift, REC RN's will administer medications and perform procedures per established policies and guidelines. This includes but is not limited to: doing daily and weekly skin assessments, monitoring and documenting pain and pain interventions on a daily and weekly basis. REC RN's will check crash carts for readiness and respond to codes as a part of the code team as needed. REC RN's will influence care outcomes by collaborating with members of the interdisciplinary team. REC RN's will participate in core elements of performance including knowledge and active participation in unit/program level quality improvement The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. The Community Living Center (CLC) Registered Nurse (RN) is an expert in providing, directing or influencing the care of Veterans, their families and significant others in a variety of settings. The CLC RN provides comprehensive nursing services independently or collaboratively with clinical and administrative staff and interdisciplinary teams. NOTE: Assignment to other areas of the service line, medical center, or other CBOC's may be required. The Registered Nurse reports to the Nurse Manager and works in collaboration with his/her assigned CORE team, however, assignments may be made to other areas for purposes of cross coverage. Work Schedule: Full-time, 80 hours bi-weekly, including weekends and holidays. There will be six (6) - 12-hour shifts and one (1) - 8-hour shift biweekly. **There are full time Night shifts available. Additional information will be provided during the interview process. There is opportunity for overtime shifts. Alternative tours of duty may be required to support operational needs on an infrequent basis. (72/80 may be available.) Telework: Not Available. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and 1 year of nursing practice/experience equivalent to Nurse 1 Level 1; OR a Bachelor's in nursing with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and 2 years of nursing practice/experience, 1 of which is a year of specialized nursing experience equivalent to Nurse I, Level 2; OR a Bachelor's degree in nursing with 1 years of nursing practice/experience equivalent to Nurse 1, Level 2; OR a Master's degree in nursing (MSN) and no additional nursing practice/experience; OR a Master's degree in a related field with a bachelor's degree in nursing with no professional nursing experience. Nurse II - A Bachelor's degree in nursing with 2 years of nursing practice/experience, 1 of which is a year of specialized nursing experience equivalent to Nurse 1, Level 3; OR a Master's degree in nursing and 1 years of nursing practice/experience equivalent to a Nurse 1, Level 3; OR a Master's degree in a related field with a bachelor's degree in nursing with 1 year of specialized nursing experience equivalent to Nurse 1, Level 3; OR a Doctoral degree in nursing with no professional nursing experience; OR a Doctoral degree in a related field with a master's or bachelor's degree in nursing with no additional nursing practice/experience required. Nurse III - Master's degree in nursing and 2 years of nursing practice/experience, 1 of which is equivalent to Nurse II and meets the dimension requirements of Nurse III; OR a master's degree in a related field with a bachelor's degree in nursing with 2 years of specialized nursing experience, 1 of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in nursing with 1 year of specialized nursing experience which is equivalent to Nurse II and meets all dimensions requirements for Nurse III; OR a Doctorate degree in a related field with a master's or bachelor's degree in nursing with 1 year of specialized nursing experience equivalent to Nurse II and meets all dimension requirement for Nurse III. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: The population of male and female Veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. Sioux Falls VAHCS is an interdisciplinary teaching facility, offering medical, surgical, and mental health inpatient services. Comprehensive health care is provided through primary care, tertiary care, and long-term care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, geriatrics, and transitional care. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring Veterans and objects may be required. The RN may be exposed to infected Veterans and contaminated materials and may be required to don protective clothing in isolation situations or operative/ invasive procedures. The RN may occasionally be exposed to Veterans who are combative secondary to delirium, dementia, or psychiatric disorders. The RN must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on Veteran needs. This position requires heavy lifting (45 lbs and over), Heavy Carrying (45 lbs and over), use of fingers, both arms/hands required or the equivalent required, walking 8 hours, standing 8 hours, repeated bending (3 hours), climbing, legs only (1 hour), ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13 to 16 to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, hearing (aid may be permitted). Environmental factors include: Fumes, smoke or gases, working with hands in water, and working closely with others.
