The incumbent functions as the Patient Safety Specialist to lead, participate, and support all aspects of the Patient Safety Program developed by the National Center for Patient Safety (NCPS). This position is the facility's key person to help build and implement patient safety systems, promote a Culture of Safety in line with a High Reliable Organization (HRO) for VA patients in all healthcare settings.
Duties
Duties include but are not limited to:
Responsible for leading, training, and directing patient safety activities to include, but not limited to RCAs, HFMEAs, Aggregates, Suicide Prevention facility assessment, Patient Safety Forums, MHEOCC inspections, EOC rounds, NPSG training and education to all personnel within the facility.
Responsible for translating the policies, theories, and strategies into action within the complex interrelationships that exist related to patient safety.
Responsibility for training and implementation of the patient safety, RCA, and HFMEA process to clinicians, management, and all relevant support personnel within their facilities and associated facilities and operations (e.g., home care).
Performs duties directly contributing to the administrative management of the facility health care delivery system, and is responsible for planning, organizing, developing, managing, and evaluating the medical center's patient safety program.
Responsible for chartering, leading, tracking and trending Root Cause Analyses from inception to completion to include following actions, and outcome measures to closures.
Serves as the facility point of contact for Salesforce Patient Safety Alerts/Advisories, including making notifications, tracking all actions in Salesforce to completion.
Plays a critical support role in developing and coordinating internal review systems to assure that both clinical and administrative patient safety activities are in compliance with agency and accrediting standards and regulatory requirements.
Maintains and monitors program requirements/validation criteria for the facility's patient safety program.
Responsible for coordination and implementation of all patient safety policies as well as providing consultation as a subject matter expert in developing service level policies, protocols and procedures that directly and indirectly focus on patient safety.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.
Work Schedule: Monday-Friday 8:00 AM - 4:30 PM

GS-11: Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-11 position you must have served 52 weeks at the GS-09. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-09 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
Ability to coordinate all Patient Safety program activities both inside and outside the service and appropriately respond to inquiries from NCPS, VISN, Executive Leadership Team, Chiefs and/or Managers and ensure issue resolutions as the final coordinator. Responsible to ensure all medical center policies related to the Patient Safety Program are updated in accordance with DCS guidelines and in line with VINS and National program requirements.
Review and revise as appropriate all Patient Safety MCP's and manages these policies through collaboration and posting.
Collaborates with the Patient Safety Manager with developing a medical center wide training program that fosters a Culture of Safety, increases patient incident reporting, and provides psychological safety for staff, Veterans and families to "see somethin, say something".
Ability to schedule and coordinate all Patient Safety related activities, to include, but not limited to, reserving conference rooms, set up TEAMS meetings, and providing accommodations for inspectors, surveyors, etc.
Responsible for participating in annual VISN QMO site survey to include preparing/providing documents, reports, and presentations, and tracking/trending all findings in collaboration with targeted services to completion and sustainment period.
Responsible for conducting medical center wide tracers focusing on mitigating patient safety risks. Uses AMP database to enter, track and trend findings and action completion.

To qualify based on education for this grade level you must have a PhD. or 3 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work.

Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond Master's degree or 2 full years of progressively higher level graduate education leading to the degree.

Track Joint Patient Safety Reporting (JPSR) actions (events still being review) Track and submit Patient Safety data for executive leadership team, committees and service.
Responsible for scheduling and coordinating all Patient Safety related activities, reserve conference rooms, set up TEAMS meetings as needed.
Responsible for creating, maintaining, and presenting the monthly PS Report as well as preparing and presenting the Annual NCPS data report.
