This position is located in the Division of Medical Services, Centers of Excellence. Incumbent serves as the Supervisor for the Centers of Excellence (COE) nursing and clinical support staff and provides clinical oversight over the following areas: Centers of Specialized Care, Infusion/Oncology, Sage Clinic, Diabetes Centers of Excellence, and Pain Clinic. Position is located in Phoenix, Arizona.

Duties

Serves as the Supervisory Clinical Nurse (SCN), exercising specific responsibility to assure that care is provided in compliance with various regulatory and professional agencies including but not limited to, the Joint Commission and American Nurses Association. Manages the unit personnel by providing adequate unit staffing (approves training, leave, and overtime requests), unit organizational functions (medical supplies, equipment and maintenance), unit safety and departmental communications (planned unit meetings and information disseminated to the staff). Clinical Care Coordination duties include: Implements and initiates disease management and health promotion guidelines to manage care for individuals, families and populations accessing the PIMC Medical Home. Collects subjective and objective health status data from multiple sources, which includes the patient, family members, team members, and documented records. Ambulatory Case Management include: managing complex referrals requiring case management as part of the ambulatory Case Management skill team. These duties include assisting with the care coordination of ordering complex durable medical equipment (oxygen, power wheelchairs/scooters) and care managing mostly un-resourced patients/referrals but may include some resourced patients/referrals. Works closely with the care team and other health professionals in the provision of health program services.

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S), GS-0610: Note: Employees currently assigned to positions in this 0610 occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. In addition to the above requirements, applicants must meet one of the following requirements, OR Completed a degree from Foreign Nursing School and have an Official certification from the Commission on Graduates of Foreign Nursing Schools. Licensure: Applicants must have active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS, GS-12:Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-11 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Applies a wide-range of direct patient care services sufficient to perform highly specialized assignments with considerable difficulty and managing complex referrals requiring case management; ability to identify areas for improvement to services, programs, and processes that ensure high quality care, performance, and nursing practices; and able to analyze health status data unique to individuals and/or diseases, and how to develop a plan of care with the care team according to clinical practice guidelines. SELECTIVE PLACEMENT FACTOR: Certification - This position requires the following certifications to be considered qualified for this position. Evidence of meeting this Selective Placement Factor (SPF) must be included in or with your application documents. Applicants will be considered not qualified if the SPF requirement is not met. You must possess and provide evidence of a current certification issued by the appropriate certifying body such as the HIV/AIDS Nursing Certification Board (HANCB) or Nation Certification Board for Diabetes Educators (NCBDE). You must meet all qualification requirements within 30 days of the closing date of the announcement.
