The Clinical Pharmacy Program Manager functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients. The incumbent serves as a mid-level provider to independently initiate, modify, or discontinue medication therapy and as a consultant for medication therapy management services.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Program Management Assists in assuring a high-quality Emergency Medicine program by serving as the pharmacist representative for the ED. Develops, conducts, and reports quality improvement monitors for clinical pharmacy practice in emergency medicine clinical practice (MUE/Research etc.) Manages an effective quality assurance program for the service that meets the changing service, patient, and Hospital Develop and maintain accurate policies and procedures for pharmacy in the ED clinical area. Acts as a liaison between the ED teams and the Pharmacy Service to solve problems, clarify orders, provide additional information needed, and to improve communications as well as serve as an emergency medicine resource. Clinical-Direct Patient Care Activities some require a scope of practice: Functions as a mid-level provider to design, implement, and monitor individualized therapeutic treatment plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. Orders, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy including changes in medication therapy, referral to primary care, or specialty provider. Acts as a liaison between inpatient and outpatient pharmacies in both the inpatient and outpatient patient care settings. Independently adjusts medication regimens based upon, laboratory, pharmacokinetic and pharmacodynamic data as available. Interviews and/or evaluates patients upon ED presentation and discharge for appropriateness of pharmacological therapy. Makes an individualized pharmaceutical care plan for each patient encountered that will be documented via progress note in the electronic health record. Clinical - Indirect Patient Care Activities Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Medication dispensing and distribution Responsible for processing all electronic orders (inpatient/outpatient) for assigned area as requested or needed including verifying patient information, correct medication dose, route, and frequency of administration. When applicable, all orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority. Education: Serves as a preceptor for students, PGY-1 and PGY-2 residents at Edward Hines Jr VA Hospital and maintains preceptor qualifications that aligns with American Society of Health Systems Pharmacists (ASHP) and set forth by the residency program director. Administration/Technical Support Work with other health care providers to develop programs for improvement of drug use. Develops and conducts medication usage evaluation in areas of need from a cost and patient outcome perspective Customer Service Routinely identifies the customer's/patient's situation properly and performs the tasks required to resolve the customer's/patient's problem accurately and in a timely manner. Ensures supervisor awareness as applicable. Follows-up, as necessary, to ensure a satisfactory resolution. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Language Proficiency: You must be proficient in spoken and written English. Education: You must be a Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure: You must possess a current, full, active, and unrestricted license to practice pharmacy in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or in the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Specialized Experience: In addition to the GS-12 requirements, you must have 1 year of experience equivalent to the next lower grade level. Specialized experience for this position consist of: Ability to effectively supervise subordinate staff; Ability to communicate orally and in writing to persuade and influence clinical and management decisions; Expert understanding of regulatory and quality standards pertaining to pharmacy; Skill in managing people or programs.
