The Clinical Pharmacist is responsible for the daily professional and administrative operations of Pharmacy Community Care Services at VA Northern California Healthcare System. Under the direction of the Outpatient Pharmacy Supervisor and in counsel with Pharmacy Leadership, the Pharmacist has full responsibility for the daily management and operation of the assigned Pharmacy section according to Pharmacy Service philosophies, policies, procedures, goals, and objectives.

The Community Care Clinical Pharmacist functions in accordance with the guidance set forth by local, VISN, National VA handbooks, directives, polices, procedures, and federal laws governing the practice of pharmacy. Duties and responsibilities include but are not limited to the following:
Clinical
Functions independently in day-to-day clinical and administrative duties associated with the position.
Uses high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the workday.
Demonstrates efficiency by working with the outpatient supervisor setting/achieving performance goal standards.
Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics, and pharmacotherapeutics.
Ensures patient medication regimen is assessed at every encounter, drug therapy for disease state managed is monitored for safety and efficacy, medication recommendations or changes are made as necessary, and consultation upon changes with therapy is provided considering patient's general medical condition and age.
Ensures adverse drug events, clinical interventions, patient drug therapy evaluations and counseling are documented in electronic medical record.
Establishes and monitors pertinent medication management system and performance improvement indicators to assure quality services are provided.
Ensures appropriate drug information is provided to patients, physicians, nurses, pharmacy staff, and other health care professionals.
Education and Training
Identifies educational priorities and makes provisions for the development and training needs of subordinates.
Develops and performs in-services promoting cost-effective medication management.
Participates in the precepting of pharmacy residents and students.
Provides and coordinates training of new pharmacy employees and re-training of others making assessments regarding capability.
Maintains general knowledge of VA, DEA, OIG, FDA, and The Joint Commission regulations.
Tracks, trends, evaluates and presents assigned metrics to staff and leadership.
Administrative
Ensures implementation of pharmacy-based actions as they relate to facility Pharmacy and Therapeutics (P&T) Committee functions and activities.
Provides written reports and/or correspondence of quality, accuracy, and completeness.
Ensures accuracy, validity, and timely submission of pharmacy workload, systematic review/performance improvement, operations, drug utilization, and other special reports pertinent to the incumbent's program area.
Maintains knowledge, skills, and flexibility to be assigned interchangeably in all areas of Pharmacy Service to provide patient care and improve operational efficiency.
Ensures protection of printed and electronic files containing sensitive information from unauthorized disclosure, loss, unauthorized alteration, or unauthorized deletion, under the provisions of the Privacy Act of 1974 and other applicable security regulations.
Actively seeks opportunities for cost savings and implements fiscally responsible interventions based on clinical and fiscal practice.
Assists Pharmacy Leadership in efficiently managing the facility's medication budget..
Formulates policies and procedures related to pharmacy functions according to VA policy and accrediting organizations.
Develops and implements appropriate procedures, maintains compliance with local, VISN, national VA requirements, policies, and procedures as well as DEA, OIG, FDA, and TJC standards.
Develops and maintains a cooperative working relationship with other departments.
Assist in the development of long and short-term goals.
Makes independent decisions, regarding the development of new programs, considering staffing and budget realities, program evaluation studies.
Program Management
Monitors, plans, and organizes daily operation of Pharmacy Community Care services.
Ensures services provided meet or exceed professional and legal standards.
Effectively organizes and controls Pharmacy Community Care.
Strives to improve operational efficiency and develops new programs.
Ensures services provided meet or exceed professional legal standards.
Assures compliance with TJC Standards, Federal Laws, OIG, DEA and VHA requirements governing control, security, dispensing and distribution of all pharmaceuticals.
Collects, analyzes, and acts upon customer service, workload, quality assurance, productivity, and workflow reports.
Maintains continuity of care to ensure the provision of prompt and accurate pharmaceutical services.
Maintains current knowledge, responsibility, and understanding of VISN, local and national performance measures.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education.
Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy).
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. 7405(c)(2)(B). The appointing official may waive the requirement of licensure for a period not to exceed 2 years for a pharmacist that provides care under the supervision of a licensed pharmacist. For grade levels above the GS-11, the candidate must be licensed. Please note that this position is at the GS-12 level only.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.
Physical Requirements. TB/Immunization Assessment and a pre-employment physical exam will be conducted by Employee Occupational Health Service.
Grade Determinations: In addition to the basic requirements for employment stated above, the following criteria must be met when determining the grade of candidates.
Preferred Experience: Two years of experience preferred.
GS-12 Outpatient Pharmacist
Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
One year of experience equivalent to the next lower grade level, or
Completion of an ACPE-accredited Pharm.D. program.
Assignments. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
Knowledge of professional pharmacy practice.
Ability to communicate orally and in writing to both patients and health care staff.
Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

The full performance level of this vacancy is GS-12. Selection will be made at the GS-12 level.
Significant Information regarding Physical Requirements: The position requires visual and auditory acuity at a level to provide safe and effective care. Use and function of all normal motor and sensory anatomy for provision of patient care, reading, writing, listening, observing/seeing, heavy lifting, bending, pulling, pushing, stooping, squatting, walking, standing, use of fine motor muscles. Must be able to sit for 3 hours, stand for 3 hours, and walk for 1 hour. The position also requires ability to work on computers for extended periods of time. Requires use of hands for actions such as typing, simple grasping, fine manipulation, and repetitive actions.
Requires the ability to frequently respond to multiple demands and priorities; emotional intelligence to respond to stressful and/or emergent situations and crises appropriately and calmly; self-care skills to maintain physical, psychological, and emotional health. Heavy lifting of 35 pounds and over, heavy carrying of 35 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously. Has potential for exposure to infectious diseases, hazardous waste and chemicals.
