Duties
This position is located in the Veterans Integrated Service Network (VISN) 12 Office, and organizationally aligned to the Patient Safety Section - Quality, Safety & Value (QSV). The incumbent will be part of a team working with the VISN 12 Patient Safety Officer (PSO) who is aligned under the VISN 12 Quality Management Officer (QMO). The Patient Safety Program Specialist is responsible for collecting, analyzing, and trending patient safety information to initiate system changes based on current concepts. The Patient Safety Specialist works closely with the VISN Risk Manager, who is aligned under the VISN 12 QMO. Risk management responsibilities include, but is not limited to, tracking, collecting, reviewing and aggregating data from the Risk Management and Peer Review Program. Assists in preparation of reports in collaboration with the VISN 12 Risk Manager to present to the VISN 12 Risk Management and Peer Review Sub-Committee, the VISN 12 Quality Council, and/or submit to the program office.
Major Duties:
Serves as a subject matter expert leading and participating in committees at the VISN and national levels to address
trends in patient safety and establish best practices for field implementation.
Participates in the implementation and maintenance of the Patient Safety Program. This includes the daily evaluation and maintenance of incident reports in the Joint Patient Safety Reporting System (JPSR).
Leads and advises Root Cause Analysis Teams and Proactive Risk Assessment teams to improve VISN Office systems and patient outcomes.
Identifies patterns and trends in patient safety data for local and network wide performance improvement, recommends areas for improvement, as well as participates in appropriate interventions/mitigation efforts.
Facilitates VISN 12 facility processes for tracking RCA action items and developing process indicators to monitor for ongoing sustainment and compliance with the standards.
Models professional conduct in routine and stressful situations and maintains all Patient Safety and Quality Assurance records.
Independently reviews analyzed data to identify and report potential deficiencies in process, product or structure and develops and implements changes which influence performance improvement.
Tracks, analyzes, evaluates trends and aggregate data, compiles reports for various levels of the organization, from
frontline to executive, and local to national VHA, as well as internal to external community partners.
Identifies opportunities for improvement and recommend subsequent actions or appropriate interventions.

Qualifications

Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-11 position you must have served 52 weeks at the GS-9 or be a current GS-11. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials.

You may qualify based on your experience and/or education as described below:
GS-11 EXPERIENCE:
Must possess at least one (1) full year of specialized experience (equivalent to the GS-9 level in the Federal government) that has equipped you with particular knowledge, skills and abilities to perform successfully the duties of the position and that is in or related to the position to be filled. Specialized experience consists of: Experience working with administrative operations including: Quality Assurance, compliance, customer service, program goals and objectives, and work processes. Gathering and compiling large amounts of information and data from wide variety of sources from local, state and national level and using analysis and evaluation techniques and methods to obtain, review, verify and report complex data. Working with Financial Management relating to budget. Developing and conducting compliance and quality assurance training for diverse audiences. Using software applications such as: MS Word, MS Excel, MS PowerPoint, VISTA, Financial Management Systems like IFCAP, and OCLS, AEMS-MERS and AutoCAD.

GS-11 SUBSTITUTION OF EDUCATION FOR EXPERIENCE:
Must possess 3 full years of progressively higher level graduate education or Ph.D. or equivalent doctoral degree which demonstrates the knowledge, skills, and abilities necessary to do the work of this position. (TRANSCRIPTS REQUIRED)

GS-11 COMBINING EDUCATION AND EXPERIENCE: Must possess equivalent combinations of education and specialized experience as described above. (TRANSCRIPTS REQUIRED)

Physical Requirements: The work is primarily sedentary, although some physical effort may be required.
