The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking a highly skilled & experienced Nurse Practitioner (NP) to serve in our Home Base Primary Care (HBPC) Service. The NP will utilize the nursing process to assess, coordinate, facilitate & deliver nursing care for patients utilizing the nursing process. Provides direct & indirect nursing care to patients & families. Accountable for adhering to established polices for standards of care & standards of practice.

Duties

The Home Based Primary Care (HBPC) program provides interdisciplinary, comprehensive health care services to Veterans in their home. HBPC patients are often older adults with multiple chronic medical conditions for whom travel outside the home to access clinic-based care may be difficult. The Nurse Practitioner (NP) is responsible for assessing, diagnosing, planning, implementing, and evaluating direct and indirect health and quality of care for patients under their care. The NP will drive a government vehicle to the homes of patients to provide them with primary care in the home setting. Some patient care may be provided virtually (e.g. via telephone or video visits). Position is located at the Los Angeles Ambulatory Care Center (LAACC) in Downton Los Angeles. Although the position is based out of the LAACC VA location it will also require seeing patients occasionally outside of this HBPC practice's area and cross-covering for other GLA HBPC providers. Required training may occur at any of the other HBPC locations (WLA, Sepulveda, Ventura). Duties include but are not limited to: - Provides direct and indirect patient care, and works as a member of a healthcare team to oversee and coordinate medical care for a population of Veterans in the given practice setting. - Takes comprehensive histories, provide physical examinations and other health assessment and screening activities, diagnose, treat, and manage patients with acute and chronic illnesses and diseases. - Orders laboratory and imaging studies and integrate the results into clinical decision making. - Prescribes medication and durable medical equipment. - Makes appropriate referrals for patients and families, and request consultations. - Aids in health promotion, disease prevention, health education, and counseling as well as the diagnosis and management of acute and chronic diseases. - Attends staff meetings, team meetings, committee meetings, and completes peer review evaluations. - Provides leadership in identifying and addressing ethical issues that impact patients and staff. - Manages program resources (financial, human, material, or informational) to facilitate safe, effective, and efficient care. - Implements educational plan to meet changing program or service needs for self and others. - Uses professional standards of care and practice to evaluate programs and/or service activities. - Consistently demonstrates positive effective customer service, communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers. - Provides active leadership in program-based performance improvement activities. - Participates in support of educational training of graduate students and other Allied trainees in related-fields. - Collaborates with others in research activities to improve care. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Nurse Practitioner: A Nurse Practitioner must be licensed or otherwise recognized as a Nurse Practitioner in a State, possess a Master's Degree from a program accredited by the National League for Nursing Accrediting Commission (NLNAC) or Commission on Collegiate Nursing Education (CCNE), and maintain full and current certification as a Nurse Practitioner from the American Nurses Association or another nationally recognized certifying body. (This certification must be in a specialty to which the individual is being appointed or selected) Preferred Experience: Must have experience working as an APRN for at least 2 years. Must be able to qualify for a full unrestricted CA driver's license. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: The physical requirements include: lifting and carrying up to 35lbs, straight pulling and pushing, reaching above shoulder, repeated bending, use of fingers, both hands required, walking (> 4 hours), standing (> 2 hours), rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other, depth perception, distinguish basic colors, distinguish shades of colors, hearing (aid permitted), work on slippery or uneven walking surfaces, work closely with others, work alone, protracted or irregular hours of work, and ability to don protective clothing in isolation situations or operative/invasive procedures. The functional requirements of this position are concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and may include requisite laboratory and other screening as required by Federal regulatory agencies.
