This position is located in the PCMC Nursing Department of the Phoenix Indian Medical Center (PIMC), within the Ambulatory Nursing Services. The incumbent is a member of the behavioral health interdisciplinary team and ensures patients receive quality patient centered care that is timely, appropriate, cost effective, sensitive and coordinated to affect desired clinical outcomes.

Duties

Psychiatric Nursing Duties 60% Provide, develop and manage clinic triage, outreach, care coordination, crisis intervention, day call, nursing coordination with the Emergency Department, and case management services for patients with Behavioral Health issues. Responsible for participating in Behavioral Health Staff meetings, Medical Staff Meetings (due to the critical nature of the service provided) and Psycho-Education activities including teaching/counseling and facilitating activities with the Behavioral Health Clinic and its patients. Developing and managing triage, gatekeeping, legal delivery of services (screens for guardianships, etc.), facilitating integrated Behavioral Health/Health/Prevention Activities, and will be encouraged to develop new flow, practice, and educational materials. Coordinates and case manages the clinical and supportive care for severely impaired Behavioral Health patients at PIMC and other Phoenix Area Service Units, contracted hospitals in Phoenix Area and tribal affiliated entities. Coordinates care for Behavioral Health patients with a focus on the Seriously Mentally Ill (SMI), coordination with community providers that specialize in our service population, and coordination with inpatient units for transition from inpatient to outpatient care. Collects subjective and objective health status data from multiple sources, which includes the patient, family members, team members and documented records. Analyzes health status data in terms of the unique individual and/or disease related situation. Develops and maintains patient registries based on disease, health condition and/or health prevention initiatives. Ensures complete pre-visit planning for patients and coordinates referrals and outside services received or needed by patients. Evaluates the outcomes of patient care and modifies the plan of care based on outcomes, individually or with the team. Evaluation includes documentation of the reassessment of patient status, evidence of patient and family participation and responses, and the rationale for the revisions in the plan of care. Initiates therapeutic interventions and administers medications as prescribed by medical providers. Utilizes standards of care when providing self-care advice, referring to primary care provider or specialist, making an appointment or instructing the patient to seek emergency assistance. Participates in staff meetings and performance improvement studies/activities. Attends relevant in-service and education programs. Applies current nursing concepts and findings from research and/or studies to practice. Recognizes urgent/emergent patient and clinic situations and initiates emergency medical procedures and complies with external disaster, internal disaster, bomb threat and fire plans and drills. Assists with obtaining lab results, medical tests, discharge summaries and other information within the hospital and other facilities and assists with arranging appointments, procedures and discharge planning. Collects suicide data for the Methamphetamine and Suicide Prevention Initiatives (MSPI) and assists with other department initiatives, as well as facility clinical quality measures, including such initiatives like QAPI or GPRA. Case Management Duties (40%) Intradepartmental Triage: Screens and provides vitals on patients in walk-in clinic to clearly determine urgent need and best fit to provider and identifies those in need of case management. Receives referral information from PIMC staff and from other service units concerning referral patients. Comprehensive Assessment: Completes an assessment or coordinates patient assessments with the interdisciplinary team of the patient's condition and situation, mental/emotional/behavioral status, environment, finances and support system. Synthesizes information and determines the needed services. Service Planning and Implementation: Develops with the patient/family and interdisciplinary team (including other Service Units as appropriate) members for a care and service delivery plan based on needs identified and available provider and financial resources. Obtains needed services for patient through arrangements with service providers. Identifies gaps in available community services and resources. Serves as a resource person on issues concerning case management. Monitoring and Coordinating: Reviews charts and care plans and contacts patient/family members and service providers as necessary to determine if services are being delivered as planned and if services are meeting needs.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Education - A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure - Applicants must have passed the National Council Licensure Examination. In addition, applicants must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Additional License Requirements for Nurse Practitioner Positions: Must be licensed or registered by a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States in the clinical specialty required by the position. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: GS-11: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Provides direct psychiatric services to patients across the life-span by assessing plans and providing support, comprehensive and therapeutic/psychiatric, and psychological health care to individual geriatric, adult, adolescent and pediatric patients and their families with as-needed consultation with the medical staff; conducts evaluation and diagnosis of psychiatric medication needs.
