This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff in the facility and the Community Based Outpatient Clinic.

Duties

NOTE: This position involves a multi-grade career ladder. The major duties listed below represent the full performance level of GS-09. At the GS- 07 grade level, you will perform assignments of a more limited scope and with less independence. You will progressively acquire the background necessary to perform at the full performance level of GS-09. Promotion is at the discretion of the supervisor and is contingent upon satisfactory performance, availability of higher level work, and availability of funds. Major Duties: Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Facilitates Whole Health educational Groups; participates in weight management, smoking cessation, diabetes, nutrition stress management, chronic pain, mental health, and other inpatient and outpatient groups as appropriate. Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veterans goals, values, and strengths. Provides guidance and offer resources from the community and nationally recognized authorities to equip Veterans with the knowledge and skills needed to improve health-related outcomes. Conducts outreach activities and contacts eligible participants for enrollment and explain the health coaching process. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Conducts interventions for Veterans using one-on-one, couple, or group modalities via in-person care, telephone or virtual video conferencing Understands health coaching structure, process, skills and techniques. Interprets knowledge of current research literature related to health and wellness, including behaviors that promote wellness and those that are detrimental. Manages Personal Health Inventory, Personal Health Plan, and components of health and well-being in Whole Health. Utilizes interpersonal and communications skills to help veterans setting health goals. Conducts presentations to educate veterans, providers and community partners on whole health approach to health care. Utilizes problem solving skills and sound business acumen to promote the growth of health programs. Coordinates care with Veterans' health care team to ensure optimal patient care. Individual Occupational Requirements (Basic Requirements) Possess a degree: behavioral or social science; or related disciplines appropriate to the position. (TRANSCRIPTS REQUIRED) -OR- Possess a combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. (TRANSCRIPTS REQUIRED) -OR- Possess four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. -AND- In addition to meeting the basic requirements above, you must also meet the minimum qualification requirements for this position. Minimum Qualification Requirements for GS-7 Superior Academic Achievement at the GS-7 level:(SAA) at the baccalaureate level in the coursework detailed above is fully qualifying. Applicants who wish to apply under this provision must submit proof from their educational institution that they meet one of the criteria listed below. To qualify for SAA, applicants must have completed all the requirements for a bachelor's degree in is qualifying for the position to be filled, and meet one of the following: Rank in the upper one-third of your class in the college, university or major subdivision. A grade-point average of "B" (a GPA of 2.95 or higher out of a possible 4.0) for all completed undergraduate courses, or those completed in the last two years of undergraduate study. A grade point average of "B+" (a GPA of 3.45 or higher out of a possible 4.0) for all courses in your major field of study, or those courses in your major completed in the last two years of undergraduate study. Membership in a national honor society (other than freshman honor societies) recognized by the Association of College Honor Societies. Education cannot be credited without documentation.) -OR- Possess at least one full year of graduate level study or possess a master's or Ph.D. in a field as stated above that provided the knowledge, skills, and abilities necessary to do the work. (TRANSCRIPTS REQUIRED) -OR- Possess at least one year of specialized experience equivalent to the GS-5 grade level in Federal service. Specialized experience would typically include, but are not limited to: working as a health coach using appropriate structure, process, skills and techniques; supports a patient's plan for health and well-being and provide information and resources for achieving goals; uses current research literature related to health and wellness; and uses creativity to engage employees and community partners in a newly developed program.. -OR- Possess a combination of successfully completed graduate education and experience to meet total qualification requirements for the grade level. To be computed by first determining the total qualifying experience as a percentage of the experience required for the grade level; then determining the education as a percentage of the education required for the grade level; and then adding the two percentages. The total percentages must equal at least 100 percent to qualify an applicant for that grade level. (TRANSCRIPTS REQUIRED) -AND- In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position. Physical Requirements: Regular and recurring work is mostly sedentary involving sitting at a desk, although the position requires the ability to move from one geographic location of the facility to another. Some physical effort such as standing, walking, bending or prolonged sitting may be required. The work places no special physical demands on the employee.
