This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The Miltary2VA (M2VA) Program Coordinator is assigned to the VA Medical Center and provides services to Veterans throughout the South Texas Veterans Health Care System (STVHCS) catchment area. Serves as the subject matter expert (SME) on the needs of transitioning Service members and recently separated Veterans. Develops procedures for and ensures the implementation and achievement of the provision of social work services in accordance with VHA Directive 1010 The Case Management of Transitioning Service members and Post 9/11 Era Veteran and VHA Directive 1011 Department of Veterans Affairs Liaison for Healthcare Serving Military Treatment Facilities and Public-Private Partnerships. Develops and implements a system to identify transitioning Service members and recently separated Veterans served by the facility and those in the catchment area not currently seen. This may involve use of a M2VA Registry, Patient Treatment File searches, the Care Management Tracking and Reporting Application (CMTRA), Veterans Health Information System and Technology Architecture (VISTA) and Computerized Patient Record System (CPRS) records, Northeast Program Evaluation Center (NEPEC) data reports and/or Classification reports, Patient Data Exchange, Network Health Exchange, contacts with the Veterans Integrated Service Network (VISN) and VA Central Office, and contacts in the community. Ensures all transitioning Service members and recently separated Veterans are screened for case management services particularly those who are severely injured or ill and require intensive care services. Assigns tasks to M2VA staff and manages workflow of the program. Acts as a primary Point of Contact and entry point for referrals from Military Treatment Facilities (MTF) to the STVHCS VA. Develops procedures for referrals between the VA medical center, Military Treatment Facilities (MTF) and/or community hospitals that promote appropriate and timely inter-facility transfers. Serves as the facility point of contact for all inquiries regarding the seamless transition of care for recently separated Veterans. Also serves as the point of contact for transitioning Service members and Veterans seeking health care from the facility. These duties include outreaching a wide variety of stakeholders (e.g., Veterans, families, state and community agencies, Veterans Service Organizations (VSOs), military support programs, etc.). Oversees the Transition Patient Advocate, including monitoring their caseload and the advocacy services provided to M2VA service members and/or Veterans in their caseload. The incumbent is responsible for management of MTF referrals and the list of case managed Veterans located in the facility's catchment area. This includes entering, editing, compiling, analyzing, and updating of pertinent data. Ensures registration of appropriate Veterans for clinical, administrative, and outcome purposes. Manages the training for all staff involved with the registry. Assists in coverage/staffing needs including outreach calls when needed. Establishes local Standard Operating Procedures (SOP)s to ensure compliance with national directives. Prepares program reports for VA Central Office, VISN, and local medical center leadership. Responsible for program management and evaluation. Establishes and monitors performance and outcome standards for the facility M2VA Program that promotes quality and efficiency of services in coordination with the overall goals of the medical center and those of the VISN and VACO. Monitors documentation to ensure that staff completes and follows the protocols for Suicide Risk Screening to include completion of the C-SSRS, Comprehensive Suicide Risk Evaluation, Suicide/Overdose Behavior Reports, and safety planning on all patients with suicidal ideation and/or on any patient whose record is flagged "High Risk for Suicide." Provides supervision and oversight to assigned social work employees. The incumbent will be required to travel to offsite clinic(s)/location(s) to provide direct supervision and conduct field visits of supervisees and engage with stakeholders for the programs to which social workers, under the direct supervision of the incumbent, are matrixed. Incumbent is responsible for staff development, and employee satisfaction. Supervisor engenders motivation and process improvement opportunities for their staff. Supervisor advises the Service leadership on process improvement activities for staff development and employee engagement. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Social Work Program Coordinator must be licensed or certified by a state area of specialized social work practice and at the advanced practice level which included an ASWB advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advance practice level. Social Work Program Coordinators must be able to provide supervision for licensure. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area.
