The Care Coordination & Integrated Case Management (CC&ICM) Social Work Coordinator is responsible for implementation, coordination, and management of all administrative and clinical aspects of the CC&ICM framework within the Southern Arizona VA Healthcare System. The CC&ICM framework is a specialized, collaborative practice among multiple healthcare professions which provides structure and standards to support inter-professional collaboration throughout the continuum of care.

Duties

Major duties include: Implements, coordinates, and manages all administrative and clinical aspects of the CC&ICM framework within the Southern Arizona VA Healthcare System, in coordination with a Nursing CC&ICM counterpart. Develops and implements policies and procedures specific to Southern Arizona VA Healthcare System and ensures compliance with local facility, VISN, the Associate Director of Patient Care Services (ADPCS), Care Management & Social Work Service (SWS), VA Central Office (VACO) directives and handbooks, as well as Joint Commission, and other accrediting bodies and regulatory standards as needed. Ensures full collaboration with nursing counterpart, for all planning, coordination, development, implementation, monitoring, and evaluation of the CC&ICM operations. Manages, monitors, evaluates, and reports on all CC&ICM related data to fully capture the day-to-day functioning and practice impacts on the facility and Veteran care. Plans, organizes, and directs CC&ICM operations, in conjunction with the CC&ICM nursing counterpart, the Supervisor designated, and the Chief of SWS. Identifies and tracks potential areas where care coordination is taking place to include but not limited to primary care, surgical care, mental health, community care, Emergency Department (ED), specialty clinics, geriatrics, suicide prevention, the Compact Act, and homeless programs. Ensures integration with the facility level care coordination and case management teams to include but not limited to Nurse and Social Work navigators, care coordinators, care managers, and case managers of, both inpatient and outpatient for seamless continuity of care. Develops, implements and tracks a facility wide CC&ICM education plan to key stakeholders to include Service Chiefs, supervisors/managers, frontline staff, and Veterans with plan for ongoing sustainment. Ensures that CC&ICM follows policies and standard operating procedures for evaluating and mitigating suicide risk to include Completion of the Columbia Suicide Severity Risk Scale (C- SSRS), Comprehensive Suicide Risk Evaluation, Safety Plan, and Overdose Report as clinically indicated. Documents social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand-off when transferring of patient care. Actively participates in the treatment planning process with other disciplines, including involvement in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that are timely and appropriate. Maintains data and statistical compilations to comply with accreditation entities, CC-ICM measures and metrics and VA and medical center policy and procedures. Provides case management to Veterans and families, as needed, throughout the continuum of care and across care locations and transitions (inpatient, outpatient, community). Provides a full range of social work services within commonly accepted standards of social work practice which includes comprehensive biopsychosocial assessments, social determinants of health screenings, Advance Directives, Goals of Care, case management, crisis intervention, risk assessments, safety planning, abuse/neglect/exploitation reporting, psychoeducation, discharge, and other services as appropriate. Develops, maintains effective professional working relationships with key medical center staff and community agencies to enhance the scope and functioning of Veterans assigned. Adheres to and active participates in a High Reliability Organizational (HRO) practice. incorporates the principles of inclusivity, Diversity, Equity and Access (IDEA) in clinical care, customer service and program development. Qualifications

Basic Requirements: Citizenship: Citizen of the United States. Licensure: Candidate must be licensed or certified by a state at the advanced practice level and must be able to provide supervision for licensure. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Grandfathering Provision. The following is the standard grandfathering policy for all title 38 hybrid qualification standards. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation. II-G39-4 September 10, 2019 VA HANDBOOK 5005/120 PART II APPENDIX G39 All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure or certification that are part of the basic requirements of the occupation. For employees who do not meet all of the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Such employees in an occupation that requires a licensure or certification, may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If an employee who was retained in an occupation listed in 38 U.S.C. § 7401(3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. NOTE: If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS 0185 social work series, they lose the grandfathering protection. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Grade Determinations: GS-12 Social Worker (Program Coordinator): In addition to the basic requirements, applicants must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: Physical demands of the work are generally minor and are generally sedentary in nature but may require occasional travel. Physical activity involves sitting, standing, walking, bending, stooping, climbing stairs, and carrying light items such as papers, books, or other supplies. The incumbent uses a computer and standard office equipment to complete tasks. The emotional demands can be stressful in working with the Veteran/Patient population and their families/caregivers. A level of self-awareness and emotional intelligence is essential. As needed, the incumbent drives GSA vehicles safely off station to various locations, such as other VA locations, community agencies, patient homes, etc. Transportation of other staff and/or patients may be needed occasionally. The incumbent must possess a valid current driver's license.
