This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The Veterans Comprehensive Prevention, Access to Care, and Treatment Act of 2020 (COMPACT) Act, Section 201 expands the Department of Veterans Affairs (VA)'s ability to furnish and pay for treatment of eligible individuals in acute suicidal crisis. The Social Worker Program Coordinator serves as the COMPACT Act Coordinator and has responsibility as the primary navigator for Veterans receiving care under COMPACT Act for the Hershel "Woody" Williams VA Medical Center (HWWVAMC). The HWWVAMC is a 1c high-complexity teaching facility with health care services at 6 locations, serving southwestern West Virginia, southeastern Ohio, and eastern Kentucky. Facilities include the Hershel "Woody" Williams VA Medical Center in Huntington, W.Va., and four community-based outpatient clinics located in Lenore, W.Va., South Charleston, W.Va.; Gallipolis, Ohio; and Prestonsburg, Kentucky. In addition, services are provided by a Community Resource and Referral Center in Huntington, W.Va.
As the primary navigator for Veterans receiving care under COMPACT Act the incumbent is responsible for coordinating transitions in care for Veterans receiving acute suicide care under COMPACT Act between varying venues of care, types of care (medical and mental health) and location of care (direct care and community care) in collaboration with assigned care coordinators at each step of the Veteran journey. The Social Worker Program Coordinator will be instrumental in providing connection and referral to assist with proper evaluation for eligibility in other programs and is the facility subject matter expert on the COMPACT Act.
FUNCTIONS OR SCOPE OF ASSIGNED DUTIES:
The Social Worker Program Coordinator is responsible for the implementation, coordination and monitoring of the COMPACT Act at the facility. Assigned duties include:
1) Utilize program coordination skills to develop and monitor administrative and clinical plans for Veterans receiving COMPACT Act services.
2) Oversees daily operations of the program and provides reports and metrics to facility, VISN and VACO as requested.
3) Captures and analyses local data trends to inform program development and improvement.
4) Collaborate with local facility revenue and finance departments.
5) Advocate for Veteran, ensuring they do not receive bills for COMPACT related care.
6) Provide education and training on COMPACT Act to internal and external stakeholders as the facility subject matter expert.
7) Reviews and develops policies, standard operating procedures, practice guidelines for the program.
8) Coordination between health care providers and care managers in direct care and community care.
9) Collaborate with inter-professional teams within the VA healthcare system and community to navigate complex care coordination.
10) Consultation and negotiation across program areas to ensure appropriate resources are secure in a clinically relevant and timely manner.
11) Coordinate with local Patient Advocate Tracking System - Replacement (PATS-R) COMPACT clinical review teams.
12) Coordinates with local facility Office of Integrated Veteran Care (IVC) Care Coordinators.
13) Provides training, education, orientation and guidance on clinical program area.
14) Provides clinical social work assessment, intervention, treatment and follow-up, using a social determinant of health framework.
15) Assists with navigation between varying levels of care and conducts comprehensive transition planning.
16) Utilizes designated COMPACT Act templates and coding requirements when documenting in the medical record.
17) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area.
18) Ability to supervise multidisciplinary staff {where appropriate}. 19) The incumbent will serve as a backup for case management needs within the suicide prevention program.
20) Other duties as assigned.

Qualifications
Basic Requirements: The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following:
Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.
Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
