The clinical pharmacy practitioner serves as a pharmacy representative for the West Palm Beach VAMC and as a special consultant for Acute Care at the West Palm Beach, FL, Medical Center.

Duties

Works independently under a scope of practice, providing medication management for those patients who are admitted to the inpatient wards. Serves as a mid-level provider who functions to initiate, modify, or discontinue medications (excluding controlled substances) as clinically indicated in the treatment of acute care/critical care disorders. Provides appropriate selection of drug therapy based upon relevant lab values and the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate. Designs a therapeutic regimen or plan, based on patient-specific information that includes establishing a therapeutic endpoint, electing an appropriate drug and related therapy, and establishing an appropriate dosage regimen. Monitors and adjust drug levels with narrow therapeutic index Monitors and assesses therapeutic and adverse effects of drug therapy, including drug interactions, through selection and evaluation of physical and laboratory parameters. Order lab tests as appropriate. Serves as the pharmacy representative or provides coverage for the Nutrition Support Team which includes ordering and adjusting TPNs and follow up care. Performs medication reconciliation of patient's profiles at time of admission to and discharge from the hospital and at every transition of care. Coordinates discharges and provides/document discharge medication counseling for assigned discharged patients. Reviews and evaluates requests for non-formulary and/or restricted medications for appropriateness and compliance with established criteria where applicable. Reports Medication Errors and Adverse Drug Events within 24 hours of their occurrence into the VA Adverse Drug Event Reporting System (VA ADERS). Serves as mid-level provider for other clinical pharmacy practitioner for scheduled and unexpected call outs in acute care and critical care areas. Provides proactive consultation to physicians, nurses and other health care providers on appropriate drug therapy. Rounds with the multi-disciplinary team on assigned inpatient unit. Qualifications

Basic Requirements: Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Open to pharmacy residents who will be graduating by July 1, 2024. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: Pharmacist - Clinical Specialist, GS-13 (Above Full Performance Level) Experience. In addition to the basic requirements, candidates must have 1 year of specialized experience equivalent to the next lower grade level (GS-12) to qualify for the GS-13 position. AND; Knowledge, Skills, and Abilities. To be creditable, the experience must demonstrate the following knowledge, skills and abilities (KSAs): Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Highly desirable for the candidates to have ASHP Accredited PGY1 Residency or 2 years of practice in acute care/critical care area, or have completed ASHP Accredited PGY2 residency and have a minimum one year experience in critical care. Highly desirable to have Board Certification in Pharmacotherapy, Critical Care Pharmacy and/or Nutrition Support Pharmacy.
