The Clinical Pharmacist Practitioner (CPP) serves as a licensed professional for the Smoking Cessation and Pulmonary Clinic at the West Palm Beach VA Healthcare System (VAHCS) with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics.

Duties

The primary function of the CPP is to assure the safe and appropriate use of medications, and to be an advocate of rational drug therapy through the following: evaluation of the appropriateness of drug therapy based on patient specific factors; individualization of drug therapy; assessing and planning treatment, managing medications, and providing drug information. The CPP has responsibility to develop, organize, and manage within the Smoking Cessation Program and Pulmonary Clinic. The CPP is responsible to serve as preceptor for Doctor of Pharmacy candidates and Post Graduate Pharmacy residents. Other primary duties of this position include but are not limited to: Clinical Works independently under a scope of practice, providing medication management for those patients who are consulted for pharmacotherapy service for Smoking Cessation and Pulmonary Clinic. Serves as a mid-level provider who functions to initiate, modify, or discontinue medications (excluding controlled substances) as clinically indicated in the treatment of smoking cessation and pulmonary disease states which include but not limited to prescribing of medications for the following: smoking cessation and COPD Work collaboratively with the patient, physicians, nurses, and other health care providers to achieve positive patient centric outcomes. Design a patient specific therapeutic medication regimen based on the patient interview, medication history, medical history, and evaluation of a medication pharmacokinetics and pharmacodynamics, side effects, and drug interactions. Order necessary laboratory tests and other tests as deemed appropriate to provide optimal medication management. Document the pharmaceutical care plan in the patient's Computerized Patient Record System (CPRS). Serves as an authoritative information source on drugs and their utilization in therapy, with special emphasis on medications used for smoking cessation and pulmonary care. Evaluates the drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs, with a special emphasis in medications used in smoking cessation and pulmonary care. Participates in multidisciplinary team meetings as assigned. Provides clinical telehealth services, such as VA Video Connect (VVC) for appropriate patients. Implements Medication Use Committee policies including therapeutic interchanges and automatic substitutions to include appropriate patient and provider education. Collaborates with the other clinical specialists in the provision of specialized training programs and assists in the development of innovative training concepts aimed at improving the knowledge and quality of pharmacy and other clinical service staff. Administrative Functions Participates in Medication Use Committee activities when requested. Reviews smoking cessation and pulmonary medication usage in patients as part of the overall quality assurance program and prepares necessary reports as required. Designs, implements and supervises the clinical education and training program for Doctor of Pharmacy candidates and Post Graduate Pharmacy practice residents. Participates in the medical center Ongoing Professional Practice Evaluation (OPPE). Register in Electronic Florida Online Reporting of Controlled Substances Evaluation (E-FORCSE) to participate in the Florida Prescription Drug Monitoring Program if indicated. May serve as a director or associate director for a post graduate pharmacy residency program based on qualifications. Project Skills Independently designs, secures funding and conducts clinical projects within the scope of pharmacy practice as appropriate. Actively collaborates with other healthcare providers as necessary in the design and implementation of protocols and collection of data and participates in the analysis and interpretation of the results. Basic Requirements: To qualify for appointment to this position, all applicants must possess the following: United States Citizenship: Citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education-Transcripts Supplied 1. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. 2. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). Licensure-Documentation Submitted Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. To qualify for this position, applicants must meet all requirements by the closing date of this announcement. Clinical Pharmacy Specialist, GS-13. Experience. One year of experience equivalent to the next lower grade. Knowledge, Skills and Abilities. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Assignments. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Preferred Experience: ASHP accredited PGY1 residency or in the process of completing a PGY1 Residency by June 30, 2025 or have at least 1 year of clinical experience with a scope of practice and have a Board Certification in Pharmacotherapy Reference: For more information on this qualification standard, please visit https://www.va.gov/ohrm/QualificationStandards/. The full performance level of this vacancy is GS-12. Physical Requirements: Work is performed in the pharmacy as well as patient care areas where there is potential for exposure to communicable disease. Work areas are adequately heated and ventilated, and normal safety precautions are required. This position operates in a work environment that is subject to frequent, abrupt, and unexpected changes in work assignments due to shifting demands, priorities, and deadlines, which require the employee to constantly adjust operations under the pressure of continuously changing and unpredictable conditions. This position functions in an operation in which work processes and procedures vary constantly because of the impact of changing technology, creating a requirement of extensive training and guidance of the subordinate staff.
