This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The incumbent will assist with teaching trainees from the University of Tennessee Health Sciences Center and other approved affiliated programs. The GeriPACT NP will participate in GeriPACT team meetings (huddles) to discuss clinical and administrative issues, specific patient care problems and professional staff needs, offering solutions to problems and identifying areas where policy should be developed. He/she keeps the GeriPACT team apprised of nursing care advances and practice standards. The incumbent joins other GeriPACT providers in various medical administrative support activities such as reviewing cases, completing signing forms not requiring physician signature for the GeriPACT patients. He/she is readily available to the team members for collaboration when medical and/or other problems arise. The GeriPACT NP also serves as backup for other GERIPACT providers. The Nurse Practitioner for GeriPACT, promotes standards of clinical competence and conduct for staff which are maintained through: compliance with credentialing and privileging processes, monitoring and evaluation of clinical practice, peer review, consideration of clinical performance matters in clinical privileging, and other appropriate performance improvement activities. The Nurse Practitioner of GeriPACT serves as a licensed independent provider (LIP) within GeriPACT clinic to provide direct primary care to assigned elderly veterans while insuring continuity of care. Geriatrics service Nurse Practitioners are independent care providers who are skilled in the management of a patient's primary care needs as well as geriatric subspecialty evaluation and treatment. Screening patients for clinically appropriate referral to other specialties for consultation is also a requirement. The incumbent will remain up to date with the latest medical treatments and procedures and applies existing body of knowledge in gerontology as well as the standards of nursing practice to increase the quality of care to the older adult. Clinical duties include: 1. Serves as a contact person and day to day clinical resource for matters involving Geriatric outpatient services. 2. Active participation as a member of the interdisciplinary team 3. Serves as alternate provider with affiliated GeriPACT programs including Memory Clinic and /or the Falls reduction clinic. 4. Provides comprehensive care for our veteran population such as preventive screenings, diagnosis, care, and treatment of assigned patients. This includes timely completion of medical documentation. 5. Provides effective medical treatment, consultation, and diagnosis related to geriatric issues upon request from other Services/Departments 6. Orders lab tests, x-rays, EKG's and other diagnostic procedures 7. Initiates or expedites requests for consults, special tests, or studies. This includes both GeriPACT patients and veterans without PCPs in the community nursing home and Memphis VAMC affiliated State Veteran Home programs. 8. Exercises accountability for the patient's rights and autonomy while delivering care according to nursing customer service standards. 9. Ensures the provision of primary health care and services to women geriatric veterans appropriate to their specific gender related conditions and needs. 10. Demonstrates the ability to assess, plan and evaluate the geriatric patient's plan of care: a. Establishes a therapeutic relationship with the patient/family to facilitate collaboration in the provision of care and services. b. Recognizes age -related changes based on an understanding of physiological, cultural, social, psychological and spiritual functioning. c. Participates with patients/families and other health care professionals in ethical decision making which is patient-centered, empathetic and humane. d. Educates patient/family regarding measures to promote health and comfort. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected. Preferred Experience and Certifications - VA Experience (CPRS), clinic management of elderly patients with multiple chronic diagnosis. Outpatient care in a clinic situation. Certification in Geriatrics. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Physical Requirements: It is VA policy to ensure that all selected applicants and employed personnel are physically, mentally and emotionally fit to perform the duties of the position to which assigned. A pre-placement physical examination is required for all fulltime, part-time and intermittent employees identified by VA Directive and Handbook 5019. The pre-placement physical examination shall be completed prior to appointment to determine the physical and mental fitness for candidates for appointment in VA. The examination will be concerned with the mental and physical ability of the applicant to satisfactorily perform the duties of the proposed assignment and will include requisite laboratory and other screening as required by Federal regulatory agencies. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. This position requires that you are able to lift at least 50 lbs. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on organizational needs. Must complete annual Employee Health requirements, such as annual Influenza vaccination, TB screening or testing, as a condition of employment.
