The Pharmacy Supervisor (Ambulatory Care) is responsible for providing pharmaceutical care to patients as assigned within the VA Loma Linda Healthcare System (VALLHCS). The pharmacy supervisor functions at the highest level of clinical pharmacy practice to provide pharmaceutical care to patients and optimizes patient care outcomes, working independently under their scope of practice as defined by the medical center to directly care for patients.
Duties
Duties include but are not limited to:
Manages specialized pharmacy service line or pharmacy location at VALLHCS, holds a position of high visibility, represents facility efforts to improve medical care, excellent communication and interpersonal skills.
Ensures exercise initiative and collaborate effectively with clinical and administrative staff at VALLHCS.
Maintains, monitor and correct problems of workflow within assignment areas.
Performs patient care activities to include planning and implementation of Clinical Practice Guidelines of drug therapy pertinent to the specialized pharmacy service line, monitoring and follow-up of therapy through appropriate laboratory tests, diagnostic tests and other supportive care modalities.
Develops policies and procedures leading to the safe and cost-effective use of medications in the specialized pharmacy program, identified in section 10 and addendums.
Provides patients cared through a multidisciplinary team approach involving physicians (attendings, residents), clinical pharmacy specialists (CPSs), nursing staff, case managers, dieticians, care managers and social workers.
Performs documentation in the medical record, recommendations, concerns and potential medication related issues to communicate these concerns to other health care providers.
Employs current VA, The TJC, Drug Enforcement Agency (DEA), Food and Drug Administration (FDA), and other research accrediting bodies or authorities to meet standards and/or regulations set forth by these organizations regarding drug distribution, compounding of sterile products, security and inventory management system and procedures in a Health Care Services Pharmacy setting.
Serves as a member of committees and task forces as assigned.
Consults and advises pharmacy management on all matters relating to research pharmacy practice, and works closely with them and their staffs to ensure that programs reflect the mission and priorities of the VHA and VA.
Provides direct patient care, and will be qualified and authorized to provide services in support of primary care, specialized ambulatory care clinics, as assigned and as outlined in the Scope of Practice.
Demonstrates the knowledge and ability to assess data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their specific needs and to provide care needed to achieve positive patient outcomes.
Provides medication management of chronic diseases as described in their Scope of Practice including medication initiation, dose titration or adjustment, discontinuation, assessment and therapeutic monitoring, patient education and medication adherence enhancement.
Provides medication management, as requested by provider consult that is consistent with their Scope of Practice, including, but not limited to, the management of anticoagulants, antibiotic stewardship, vaccinations, and discharge medication coordination.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-13 (Pharmacy Supervisor) Experience: Applicants must have at least 1 year of experience equivalent to the next lower grade level (GS-12). In addition the candidate must demonstrate the following KSAs:
Ability to effectively supervise subordinate staff.
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards pertaining to pharmacy.
Skill in managing people or programs.
Assignment. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area.
Preferred Experience: 1 year supervisory experience.

Physical Requirements: This may require lifting and carrying light loads, including boxes, equipment, unit-dose cassettes, and IV solutions; stooping or kneeling (for example, to pick up items from the floor, remove and replace items on lower shelves, and to file documents in lower file drawers, ect.); sitting, walking, or standing for long periods of time. Normal ranges of hearing and vision are required to perform work and communicate effectively; must have the visual ability to distinguish between basic colors and shades of color. An ability to perform tasks requiring rapid mental and muscular coordination functions simultaneously is necessary. Must be physically capable of operating various types of equipment used for the job.
