This position is located in Social Work Service, Hampton VA Medical Center (HAMVAMC). The incumbent is assigned to the Home and Community Care Program (HCC) and serves as the Program Specialist. The primary duties of the Program Support Assistant (PSA) are program development, data collection, analysis of trends, and implementation of best practices.

Duties

Duties: The SW Home and Community Care program specialist is an integral member of the Interdisciplinary Management Team as needed in the delivery of care for patients receiving Non-Institutional Care services from the Hampton VA Medical Center. The primary duties of the position involve oversight of the department's data capture and coordination processes, including special projects as assigned and/or appropriate, data tracking and reporting, consult tracking, including encounter and provider documentation completion. The primary reason for the position is to assist the Home and Community Care Manager in the initiation, documentation, revision, and maintenance of contractual issues, invoices, data tracking and reporting, inspection records, minutes, Clinic Profile Management and specialized service level recovery. The position supports the facility's adherence to the Veterans Health Administration's Social Work regulations as pertains to Contract Nursing Homes and Non-Institutional Care. It is strategically placed in the Home and Community Care program in the Service Line and reports to the Home and Community Care Manager. The position works extensively with the computerized patient record system (CPRS) and Veterans Health Information Systems and Technology Architecture (VistA) in ensuring data is accurate and complete. Assignments typically involve all aspects of managing Social Work NIC data measurement and analysis, flowcharting of work processes, and use of statistical measures. The position tracks, compiles, and reports data in response to external requests from such sources as the GAO (U.S. Government Accountability Office), VACO (Veterans Administration Central Office, VISN (Veterans Integrated Services Networks). Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 12/29/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade (GS-7) in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: duties in the areas of Program Support, Contract Monitoring, Consult Tracking, and Clinic Profile Management. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have 2 full years of progressively higher-level graduate education leading to such a degree, or LL.B. or J.D., if related. . OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond progressively higher-level graduate education. Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: The work is sedentary. Typically, the employee sits comfortably to do the work. However, there may be some walking; standing; bending; carrying of light items such as papers, books, small parts; or driving an automobile, etc. No special physical demands are required to perform the work.
