This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The HUD-VASH Program is an intensive case management program for both chronically homeless Veterans and homeless Veterans with severe service needs. HUD-VASH is aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The incumbent in this position functions as a Team Lead for STVHCS Housing and Urban Development / VA Supportive Housing (HUD-VASH) Program.
The incumbent is responsible for providing expert guidance about issues facing homeless Veterans which includes but is not limited to mental health conditions, substance use disorders, social isolation, and financial instability.
The incumbent is responsible for the following duties/responsibilities:
Provides direct clinical social work services for HUD-VASH Veterans within the medical center and the community.
Provides assessment and specialized case management to high acuity Veterans within the HUD-VASH program. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing. Providing the appropriate level of case management and supportive services, primarily in the community or in the home, to Veterans based on their needs and wants.
Develops comprehensive treatment plans with the Veteran that address the factors that contributed to homelessness, including mental health, substance abuse, or physical health conditions within a recovery model framework.
Serves as a team lead to provide clinical guidance to less experienced GS-11 social workers on assigned team. Guides discussions on appropriate acuity levels for Veterans based on presenting issues and strategizes with team members on ways to manage challenging patients.
Serves as a Care Coordination and Intensive Case Management Lead Coordinator for Veterans within the HUD-VASH Program who require complex care coordination.
Provides clinical support to the HUD-VASH team as a functional member of this team, to include consultation, interventions, or accompanying other case managers on home visits, when indicated by concerns about mental health, deterioration of physical health, or substance use.
Provides training for other members of the multidisciplinary treatment team in Critical Time Intervention, Housing First, and/or Motivational Interviewing.
Provides liaison services with other programs referring homeless Veterans to HUD-VASH to ensure continuity of care and promote successful transition of Veterans from one program to another.
Follows STVHCS standard operating procedures for evaluating and mitigating suicide risk to include completing the Columbia Suicide Severity Risk Scale (C-SSRS), the Comprehensive Suicide Risk Evaluation, and a Safety Plan via shared template in the Computerized Patient Record System (CPRS) on all patients at intake, with Veterans with suicidal ideation and on any Veteran whose record is flagged "High Risk for Suicide." Additionally, completes the Suicide Behavior and Overdose Report (SBOR) as clinically indicated.
Practices group psychotherapy with Veterans enrolled in the program or who have graduated from case management services and utilizes time limited individual evidence based therapeutic techniques (such as Motivational Interviewing, Cognitive Behavioral Therapy) as needed with assigned Veterans.
Conducts an intake assessment with Veterans referred to the HUD-VASH program including a comprehensive psychosocial assessment with multi-axial diagnostic evaluation to determine the needs of veterans and family members as well as the acuity level of Veteran. Additionally, completing a HOMES assessment, referral and program entry.
Performs other related duties as assigned.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/ OHRM/T38Hybrid/.

English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f).
Grandfathering Provision. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Senior Social Worker GS-12
Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
