The Social Worker (Program Coordinator) serves as a program coordinator at the GS-12 grade and is administratively and clinically responsible for the Grant Per Diem (GPD) and Contract Residential Programs.

Duties

Duties may include, but are not limited to: Facilitates evaluation and treatment of Veterans in the GPD and Contract Residential facilities and participates in interdisciplinary team meetings (both at the agency and at the VA facility) with enrolled Veterans. Provides all necessary direct and indirect patient care services consistent with the area of assignment. Develops and maintains service delivery systems for homeless Veterans including but not limited to collaborating with other agencies, building and maintaining an effective treatment team, and providing structure and documentation to accomplish these tasks. Provides services and oversight of the Homeless GPD and Contract Residential funded programs as defined in the GPD and Contract Residential programs' rules, regulations, and directives including verification of Veteran status and eligibility for participation in the Homeless GPD and Contract Residential Programs. Maintains accurate records of admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care. Works with the Homeless Programs Supervisor to develop and maintain appropriate budgets, staffing patterns, monitor expenditures, obligations and other funds to assure the program remains within budget guidelines. The GPD And Contract Residential Social Worker Program Coordinator will also meet regularly with the Homeless Programs Supervisor to review progress and performance. Collects and submits participant information including completion of all HOMES relevant HOMES forms. Collect and transmit to appropriate persons/services all necessary outcome data and reports used to evaluate program performance and progress in a timely manner. Coordinates and participates in program site inspections utilizing the GPD and Contract Residential programs' rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices. When necessary, notes inspection deficiencies and establishes methods to monitor corrective actions and develop timelines for consequences as a result of inaction or insufficient actions related to deficiencies. Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans as well as provides current information on services and benefits available in the VA for Homeless GPD and Contract Residential Program participants. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the Homeless GPD and Contract Residential Social Worker Program Coordinator. Actively participates in program planning and development, advising the Homeless Programs Section Chief and Chief of Social Work of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Participates in program evaluation and performance improvement initiatives at the program, medical center, VISN and national levels by identifying questions and trends that should be studied as a basis toward improving aspects of social service and/or physical or mental health care delivery to Veterans. Devises and carries out projects designed to improve Veteran care. Actively participates in the GPD and Contract Residential conference calls and attend trainings offered that will assist in programmatic duties associated with position. Participates in other relevant conference calls and/or meetings as assigned. Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.GS-12 Social Worker Program Coordinatorcandidates must have the following: Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. These are the KSA's for this subject position: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. References: VA Regulations, specifically VA Handbook 5005/120, Part II, Appendix G39, Social Worker Qualification Standard. Preferred Experience: Homeless experience and Grant Per Diem (GPD)/Contracting experience. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive a government vehicle.
