The position is located in the Public Health Nursing section of the Division of Nursing Services at Phoenix Indian Medical Center. The purpose of this position as a Community Health Nurse (CHN) is to assess, diagnose, investigate, identify outcomes, plan, implement and administer a comprehensive public health nursing program for Phoenix Indian Medical Center and communities. Position is located in Phoenix, Arizona.
Duties
Assesses the health needs of individuals, families, and the community served taking into account the total situation in the family, the home, and in the community environment including such considerations as educational level, income level, sanitary practices and hygiene conditions, psychological adjustment, and resources available.
Interviews and observes the patient, family members, and others, observes the environment, studies the medical diagnosis and treatment plan when one has been made; and interpret test previously administered. Evaluates these factors as they relate to each other, to the patient, to the family, and to the community.
Recognizes symptoms that indicate the early stages of serious physical, emotional, or mental problems and other needs for medical care of all family members, for recording the details of such cases, and for referring them for medical follow up.
Tracks community health status, evaluates health trends and risk factors of population groups. Collects data in a systematic and organized manner, i.e. demographic, immunization, and analyzes the collected assessment data.
Investigates health problems and hazards such as communicable diseases and develops a recommended plan of action to address these issues.

BASIC REQUIREMENT(S):Individual Occupational Requirements
Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied.
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples Include: Providing highly specialized nursing care to high risk populations in the setting of clinics, homes, schools, and communities; developing health education materials, investigating health problems in the community to identify potential/outbreak situations-field epidemiology; leading and coordinating wellness clinics.
OR
Successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
