Academic Detailing Clinical Pharmacist Practitioner for VISN 5 Academic Detailing Program. The Clinical Pharmacy Specialist is located in Pharmacy Service, Martinsburg, WV.

Duties

The overarching mission of Academic Detailing (AD) is to promote evidence-based practices, build relationships with healthcare teams, and resolve barriers to improve Veterans' care through academic detailing. The Academic Detailer is forward-thinking and must possess exceptional interpersonal skills. The incumbent must possess the knowledge, skills, and abilities in advanced clinical pharmacy practice. This incumbent duties includes but not limited to: Providing direct patient care and clinical consultative services. Be involved in research activities and precepting of trainees. Engage in academic outreach visits on campaigns identified within the VISN and by national Academic Detailing Services (ADS). Academic detailing outreach will be provided to clinicians and other members of the healthcare team in individualized and small group settings. The incumbent will provide a quality-oriented academic detailing approach using advanced implementation and communication strategies to aid clinicians in engaging in shared decision-making based on balanced evidence-based medicine. Responsible for the development of clinician and patient resources as needed. Possessing basic knowledge of data tools and evaluation in order to develop, identify, and trend quality assurance metrics to align with facility, VISN and national goals. They will be responsible for managing their assigned AD program(s) which requires coordination of multiple locations such as medical center care facilities, CBOCs, rural health, etc. Responsible for evaluating their AD performance and program success through the use of appropriate monitoring and evaluation tools. Assisting clinicians to overcome clinical implementation barriers through supportive activities such as process improvement strategies. Expected to exercise initiative and collaborate effectively with ADS, VISN, and facility staff to meet performance goals and support quality patient care activities. Additionally, the incumbent may work under a scope of practice to provide direct patient care and/or clinical consultative services. Managing their assigned AD program(s) requiring coordination of multiple locations such as medical center care facilities, CBOCs, rural health, etc. Reports on the success of the program, including improved quality assurance metrics, areas of improvement and lessons learned. Seeks feedback and willing to adapt and modify priorities in line with local needs. Operationalizes academic detailing campaigns as assigned. This includes but is not limited to environmental assessment, stakeholder engagement, module preparation/upskilling, in-service/socialization, process improvement strategies, outreach visits, and evaluation. These outreach and supportive activities need to support campaign implementation and promote key initiatives that are assigned. Conducting strategically scheduled Academic Detailing outreach visits with clinicians and other members of the healthcare team for National, VISN and/or local Academic Detailing initiatives. Both one-on-one and small group meetings will be used to facilitate discussions. Detailer provides a quality-oriented approach that helps clinicians make appropriate clinical decisions, ideally based on the best available safety, efficacy, and value-based data. Training and educating designated staff to promote evidence-based medicine as it relates to national and VISN initiatives (i.e., in-services, staff development, grand rounds, and other formal education and training programs) Providing consistent follow-up to questions/requests, is available for lectures/in service presentations related to initiatives as requested by facilities and advises implementation strategies for planned activities as needed. Assisting with the development of balanced, evidence-based materials/products that can be used for detailing and other knowledge translation services. The incumbents' expertise in knowledge translation is key to moving research from laboratories, journals, and academic conferences into the hands of clinicians who can put it to practical use. Disseminating VISN created materials/products and best practices to other VISNs and/or nationally when appropriate. May include socializing new campaign messages to healthcare system to raise awareness and the roles staff have in supporting improved healthcare delivery for veterans. Being involved in research activities and precepting of trainees. Periodically travel may be required of incumbent to conduct academic detailing outreach visits to assigned facilities. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) OR, (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Scope of Practice: The individual scope of practice will be dependent upon the incumbent's education, training, and experience required to perform the functions of the position. All VHA pharmacists with a SOP that includes prescriptive authority must participate in a professional practice evaluation program. Frequent/Occasional Travel: The position requires occasional travel away from the duty station for outreach visits to outpatient clinics, local meetings, and infrequent distant meetings and conferences. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations, GS-13: Experience. This position is above the full performance level for Licensed Pharmacists. One year of experience equivalent to the GS-12 level is required. This experience would allow you competently answer the GS-12 level KSAs: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Assignment: Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: In addition to meeting the basic requirements, completion of an ASHP Accredited PGY-1 Pharmacy Residency and PGY-2 Ambulatory Care Pharmacy Residency is preferred. Consideration will be given to applicants with the equivalent of at least one year of specialized experience in Ambulatory Care Specialty Practice, Mental Health, Administration, or another Specialty area of Clinical Practice and/or have Board Certification through the Board of Pharmaceutical Specialties (BPS). The full performance level of this vacancy is GS-13. Physical Requirements: In accordance with VA Directive and Handbook 5019. The work is primarily performed while sitting, though some work may require periods of standing. The regular and recurring work of this position involves sitting at a desk, conferences, meetings, etc., and periodic visits to nationwide sites. Occasional lifting of boxes holding educational materials to be distributed at conferences, health fairs, etc., is also required.
