This position is within the Pharmacy Service of the Martinsburg, West Virginia Veterans Affairs Medical Center. The Clinical Pharmacy Practitioner serves as a provider that may have prescribing privileges as outlined in the individual's Scope of Practice for Clinical Pharmacy Practitioner (CPP). The primary responsibility of the Clinical Pharmacy Practitioner is the provision of evidence-based pharmaceutical care services through both direct and indirect interaction with patients and providers.

Duties

The Clinical Pharmacy Practitioner CPP is a pharmacist with advanced training in pharmaceutical sciences, pharmacology, and clinical pharmacy (pharmacotherapy) that functions as an independent mid-level provider. This pharmacist serves on the Long Term Care units at the Martinsburg VA Medical Center and promotes and manages drug therapy based on current clinical knowledge that is consistent with policies established at the national, VISN, and local levels, and functions as part of an inter-disciplinary team. Major Duties: Prescribes medications, devices and supplies to include initiation, continuation, discontinuation, monitoring and altering therapy, based on established formulary or protocols. Demonstrates an ability to work under a variety of circumstances to maximize group productivity quality, and is always supportive of the group's efforts. Shows appropriate and proactive interpersonal skills to achieve the goals of the service. Performs medication reconciliation and summarizes significant findings in the medical record and/or to the provider as appropriate. Involves in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required and documenting those finding and recommendations to appropriate individuals and in appropriate records. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Meets with patient's care team in to discuss, design, and implement treatment plans. Designs, implements, documents, and monitors therapeutic drug plans to achieve definitive outcomes through direct interactions with patients and providers in assigned areas. Provides patient-specific drug therapy modifications to maximize patient response and minimize drug interactions, adverse drug effects, and polypharmacy while providing cost-effective treatment. Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's medication therapy. Follows-up with patients on lab or test results to discuss the plan of therapy (i.e., changes in medication therapy, monitoring and evaluation, additional testing requirements, referral to primary care or specialty care physician). Performs the physical measurements necessary to ensure the patient's appropriate clinical responses to drug therapy. Consult with the supervising provider for any clinical practice area outside of his/her usual area of practice and/or not otherwise discussed in this scope of practice. Orders consults, as appropriate, to maximize positive drug therapy outcomes and disease state management. Provides in-services and other education to health care professionals when appropriate Serves as a preceptor for pharmacy students and pharmacy residents in accordance with established protocols. Assists in the development of proposals for improved and/or new clinical pharmacy services. Maintains a current knowledge of therapeutics and disease management. Provides timely and accurate responses to drug information inquiries from all customers using appropriate references to research drug information. Participates in a peer pertinence and peer review program as appropriate. Licensure: Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements above, applicants must possess the following grade requirements. Applicants must have one year of experience equivalent to the next lower grade level (GS-12). In addition, applicants must meet the following Knowledge, Skills, and Abilities (KSAs): Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area, or area of clinical expertise. Expert knowledge of specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Assignment: The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify, or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring, and documenting therapeutic plans. utilizing the most effective, least toxic, and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Preferred Experience Residency/Experience: PGY-1 Residency or at least 2 years of hospital or geriatrics experience preferred, but not required.
