Central Ohio VA Health Care System is seeking an RN for Primary Care for Discharge Planning/Care Coordination. Duty location is the Chalmers P. Wylie VA ACC. This RN will provide safe, prompt, and skillful nursing care for patients with a variety of needs and care management. They must also have excellent customer service skills, work well in teams and have the ability to collaborate with other services. BSN preferred, BLS Certification is required.
Duties
The Discharge Planning/Care Coordinator RN is the dependent stage of working closely with a preceptor with a goal of developing skills and judgment to utilize the nursing process in real situations.
The Discharge Planning/Care Coordinator RN delivers fundamental, knowledge-based care to assigned clients while developing technical competencies. The Discharge Planning/Care Coordinator RN responsibilities include, but are not limited to:
Coordinate discharge care with hospitals
Review and update daily hospitalization list
Review discharge
Contact overnight discharged patient to assure that their needs have been met
Contact/ alert public health nurses for all home health care referral
Review new admission ER/Inpatient
Enter/order labs for post-discharge per facility standing order
Enter order for DME per facility standing order
Place RTC order as needed
Alert discharge planner NP for new medication, consult placement order
Place a discharge note to include d/c summary
Alert PACT team for post d/c follow up.
Forward d/c summary to PACT team: Include inpatient test results as needed.
Maintain a data base of high utilization for each PACT team review and interdisciplinary
team review
Work across department to ensure quality and timely care at veteran discharge
Participate in quality care review/projects, task forces for clinical performance
improvement
Consult with utilization review team
Deploy VVC/telephone appointment with discharge patient as needed for the purpose of
medication reconciliation, education, and evaluation.
Present hospitalization trend to chief of primary care and associate chief nurse or other parties as needed
The Discharge Planning/Care Coordinator RN will adhere to all current VHA directives, and meet current Joint Commission standards, and VA-OIG expectations.
The Discharge Planning/Care Coordinator RN will intentionally create opportunities to leverage virtual care technology to support the mission of the facility and VA at large.
The Discharge Planning/Care Coordinator RN meets the needs of both the internal and external customers while supporting the mission of the VA by consistently communicating and treating customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. In addition, s/he provides customers with consistent information according to established policies and procedures and handles conflict and problems constructively and appropriately.
The Discharge Planning/Care Coordinator RN delivers fundamental, knowledge-based care to assigned clients while developing technical competencies. His/her scope of practice in the state of Ohio is outlined in Section 4723.01(B), ORC which defines the scope of registered nurse practice as: "Providing to individuals and groups nursing care requiring specialized knowledge, judgment, and skill derived from the principles of biological, physical, behavioral, social, and nursing sciences. Such nursing care includes:
Identifying patterns of human responses to actual or potential health problems amenable to a nursing regimen.
Executing a nursing regimen through the selection, performance, management, and evaluation of nursing actions.
Assessing health status for the purpose of providing nursing care.
Providing health counseling and health teaching.
Administering medications, treatments, and executing regimens authorized by an individual who is authorized to practice in this state and is acting within the course of the individual's professional practice.
Teaching, administering, supervising, delegating and evaluating nursing practice.
In addition, the Scope of Practice is defined by the dimensions outlined in the VA Directive 5102.1 Section (C.7.) and specific to the grade and level (refer to Attachment A). The dimensions of focus are:

The Discharge Planning/Care Coordinator RN reports to the Clinical Nurse Manager, Primary Care. It is the Nurse Manager or designee, who has direct control over work assignments and leave approval. The RN is expected to work independently within his/her scope of practice, according to policies, training, and accepted VA practice.
Work Schedule: Work Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m. Schedule may vary as deemed necessary to meet the needs of the Veterans and the Medical Center.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: 3-5 years of Nursing Case Management experience with large patient volumes.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. S/he may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. S/he must be a mature, flexible, sensible individual capable of working effectively in stressful situations, and able to shift priorities based on patient needs. Applicants must possess the mental and physical ability to satisfactorily perform the duties of the proposed assignment. Physical requirements may involve lifting, carrying, pulling, pushing, reaching above shoulder, walking, standing, sitting, kneeling, and repeated bending. S/he must complete annual Employee Health requirements, such as TB screening or testing on hire, as a condition of employment.
