The Western New York VA Medical Center is currently recruiting for one Social Work Coordinator within their Geriatrics and Extended Care. The Social Work Coordinator is a full-time position with administrative and clinical responsibilities within Geriatrics and Extended Care.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Social Work Coordinator Duties: - Provide administrative, technical, budgetary monitoring and oversight necessary for accomplishing the H&CBC program coordination and management functions for the VDC program. - The incumbent performs a wide range of services and provides technical assistance to State, local, and private agencies. Technical assistance provided to monitor activities and program reviews, and the development and analysis of information. In addition, the incumbent will use sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs work related to the review, monitoring, analysis, planning and assessment of each program under his/her authority as well as quality improvement aspects. Responsible for key project activities related to the planning, development, and implementation of the project component. - Initiates consultation with the Supervisor of SWS, H&CBC Medical Director and/or ECRS ACOS, as well as, with other authoritative resources at the Medical Center, such as Regional Counsel, and Fiscal Services in unusual and/or complex administrative and clinical problems. - As an advanced practitioner, incumbent serves as the Social Work Coordinator of the VDC program. Incumbent duties include all administrative oversight and clinical responsibility for all programs under responsibility. Incumbent is responsible for weekly, monthly, and quarterly meetings and reports respective to each assigned program. Incumbent is also responsible for program reports to VISN and VACO for the coordinated programs. Seeks guidance from VACO program office as needed. Develops and implements information systems to track service activities including visits, referrals, patient demographics, and patient satisfaction. - Duties include ensuring Medical Center Memorandum compliance with the VDC program. As the Coordinator, incumbent is responsible for scheduling, organizing, and inspecting of community provider facilities of each program respectively and completion of appropriate required reports and follow-up to ensure program operation. - Incumbent is responsible for negotiating, facilitating, and completing basic ordering agreements/contracts for the VDC. Responsible for the development, implementation and application of VA guidelines to ensure standards maintained promote health, safety, welfare, and satisfaction of the Veteran receiving community services; coordination of annual inspections and appropriate oversight to ensure compliance with all VA guidelines; assurance of optimal qualitative/quantitative clinical program services via interdisciplinary collaboration, program development, and effective utilization of VA/community resources to provide appropriate services and patient advocacy. - Is knowledgeable in and promotes Recovery Oriented Care Principles, Evidence Based Practice for Social Work and patient centered care practice with Social Work Staff. - Participate in committee assignments, as recommended by Social Work Leadership, and directed by Care Line Leadership. May serve on committees, work groups, and task forces at the facility, VISN, and/or VA Central Office levels. - Provides advice and counsel to workers related to work and administrative matters. - Provides coverage to other social work lead programs in Home based community care programs when directed on a limited basis. - Responsible for equal employment opportunity (EEO) statues by adhering to nondiscriminatory practices regarding race, color, religion, sex, and any other protected class. - Performs work related review, monitoring, analysis, planning and assessment of Social Work programs. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations Social Worker (Program Coordinator), GS-12: Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
