The EM CPP functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients within the Hines VA Hospital system. The incumbent serves as a mid-level provider to independently initiate, modify, or discontinue medication therapy and as a consultant for medication therapy management services within the Hines VA Emergency Department.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Duties include but are not limited to: Clinical - Direct Patient Care Activities some require a scope of practice: (1) Functions as a mid-level provider to design, implement, and monitor individualized therapeutic treatment plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. (2) Orders, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy including changes in medication therapy, referral to primary care, or specialty provider. (3) Acts as a liaison between inpatient and outpatient pharmacies in both the inpatient and outpatient patient care settings. (4) Independently adjusts medication regimens based upon, laboratory, pharmacokinetic and pharmacodynamic data as available. (5) Interviews and/or evaluates patients upon ED presentation and discharge for appropriateness of pharmacological therapy. Makes an individualized pharmaceutical care plan for each patient encountered that will be documented via progress note in the electronic health record. (6) Apply knowledge of normal laboratory values in the evaluation of patient care, recognizes significant abnormalities, and makes dose adjustment recommendations based on objective laboratory findings. (7) Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable and as assigned.(8) Supports effective drug usage criteria approved by the national VHA Pharmacy Benefits Management (PBM), VISN PBM and local Pharmacy & Therapeutic (P & T) which help to ensure quality patient care in a cost-effective manner. Ensures appropriate utilization of the VHA formulary by health care providers. (9) Develop, upgrade, and disseminate acceptable criteria for use of formulary and non-formulary medications and other high-cost items directed at employed cost effectiveness with compromising the quality of patient care. Dissemination of information may occur in a setting requiring speaking in front of a large group of providers (10) The EM CPP is responsible for responding and assisting with cardiopulmonary resuscitation (CPR) in the event of a cardiac or respiratory arrest alert unless there is a written advanced directive or a physician's order to the contrary in accordance with Policy Memorandum 578-03 111-022 Cardiopulmonary Resuscitation - (CPR). The incumbent who practices in Pharmacy Service must maintain current BLS and ACLS certifications. Clinical - Indirect Patient Care Activities (1) Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. (2) Performs medication reconciliation during transition of care (e.g., ER visit, admission, and/or ER discharge) as applicable. Informs appropriate healthcare providers of any discrepancies noted. (3) Utilizes VAADERs Adverse Drug Reaction (ADR) reporting system to monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. (4) Enters and documents medication errors or findings in the medication reporting system. Medication dispensing and distribution (1) Responsible for processing all electronic orders (inpatient/outpatient) for assigned area as requested or needed including verifying patient information, correct medication dose, route, and frequency of administration. When applicable, all orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority. (2) Does not verify, check, or release any prescription/medication order that they prescribed. Education: (1) Serves as a preceptor for students, PGY-1 and PGY-2 residents at Edward Hines Jr VA Hospital and maintains preceptor qualifications that aligns with American Society of Health-Systems Pharmacists (ASHP) and set forth by the residency program director. (2) Maintains both pharmacy and Emergency Medicine competency through continuous self-development activities. (3) Participates in the development and implementation of different education as needed. Ensures training, or in-servicing of any health care provider including but not limited to: physicians, residents, technicians, pharmacists, students, or nursing; is completed on time or as needed if required. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) OR (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS 13 Clinical Specialist Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
