This position is located within the Primary Care Service at the James E. Van Zandt VA Medical Center, a secondary care facility with 11 acute beds and a 40 bed Nursing Home Care Unit, and five Community Based Outpatient Clinics. The duties and responsibilities are delivered throughout the medical center including all clinical and other patient care areas involved with the service.

The core elements for a Certified Registered Nurse Practitioner (CRNP) Scope of Practice may include, but are not limited to:
Obtaining medical histories and performing physical examinations
Providing and coordinating medical care for assigned patients in any care setting, including establishing diagnoses, formulating and implementing care plans, and providing follow-up care
Evaluating outpatients to determine the need for further health care
Ordering diagnostic studies and other special tests, such as Magnetic Resonance Imaging (MRI), Computerized Tomography (CT) scans, etc.
Carrying out health promotion, disease prevention, and patient education activities
Ordering or obtaining laboratory specimens
Ordering ancillary services, such as pharmacy, social services, physical medicine and rehabilitation therapies, prosthetic services, etc.
Ordering VA specialty service consults and initiating requests for non-VA medical care and consults in accordance with VHA and local facility policy procedures
Ordering patient care supplies
Writing orders for or prescribing medications:
(1) CRNPs may prescribe or order all medications listed in the VA National Formulary consistent with the CRNP's level of prescribing authority and practice setting. Exceptions include medications restricted to specific clinical services outside of the CRNP's practice setting and those specifically restricted by VHA or local VA medical facility policy.
(2) CRNP's may prescribe non-formulary medications when established non-formulary drug request policy and procedure is followed.
(3) CRNP's prescriptions or orders for medications when included in their Scope of Practice do not require a physician co-signature.
(4) Controlled Substances. CRNPs may be authorized to prescribe or order controlled substances utilizing the CRNP's individual Drug Enforcement Administration (DEA) registration when:
a. The CRNP is currently certified
b. Holds a current, active, full and unrestricted license or registration from a state that authorizes CRNP's to prescribe controlled substances
c. The schedules of drugs the CRNP is authorized to prescribe by their Scope of Practice is consistent with the schedules established by the CRNP's State licensure or those included on the CRNP's individual DEA registration certificate, whichever is more restrictive
Providing clinical video Telehealth in primary care, if appropriate

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant:
(1) The National League for Nursing Accrediting Commission (NLNAC) or
(2) The Commission on Collegiate Nursing Education (CCNE).
Note: The approving official may authorize a waiver of the requirement for NLNAC or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR,
(B) The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR,
(C) In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Master's Degree from a program accredited by The National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.
Preferred Experience and Certifications - Family Practice or Primary Care
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR, a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR, a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: Physical demands include repetitive body movements including the following: frequent reaching, standing, walking, and lifting of equipment and/or patients to provide patient care. This position requires extension of the hand(s) and arm(s) in any direction including above or below shoulder level, during the provision of patient care, including retrieval of needed equipment, supplies and to make contact with the patient. This position requires the maintenance of an upright position to perform duties associated with patient care. This position is typically non-sedentary, and frequently involves being in the standing position. This position requires moving about accomplish tasks, such as assisting with patient ambulation and moving one patient area to the next. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move a patient from one location to the next, i.e.- from the exam table to the chair, and in the movement of equipment or supplies.
