The Oncology Clinical Pharmacy Specialist (CPS) will provide pharmaceutical services to Veterans managed by outpatient oncology services at the Robert J. Dole VA Medical Center. He/she will serve as a medication expert as part of a
multidisciplinary team caring for Veterans with hematology/oncology disease states, both malignant and benign. The role will focus on the pharmaceutical management of cytotoxic and non-cytotoxic medications based on national consensus guidelines and standards of care, including assessment and vetting of non-formulary medication use and will serve to educate Veterans regarding their pharmaceutical care while participating in their medication management to include, but not limited to history, disease assessment, regimen selection, supportive care, treatment response assessment and follow-up.
Clinical Practice
a) Acts as an independent provider operating under a scope of practice and providing expertise to other members of the multidisciplinary team.
b) Discusses care of oncology patients to include, but not limited to, treatment regimens, supportive care, symptom management, adherence, and posttreatment follow-up.
c) Conduct patient interviews including, but not limited to, medical history, medication reconciliation, assessment and treatment planning.
d) Documents all patient interactions and assessments in the medical record promptly and accurately.
e) Ensures safe, appropriate, cost-effective medication therapies for patients, in accordance with established policies, procedures, and protocols.
f) Reviews and interprets physicians' orders for appropriateness of medication ordered as it relates to the patient's diagnosis.
g) Assesses patient's medication profile for drug sensitivities, incompatibilities, allergies or drug interactions.
h) Assists the oncology team in developing and implementing order sets to help guide clinical care to safe, effective and cost-effective treatment of oncology patients including supportive and follow-up care.
i) Assesses and responds to clinical consults related to oncology and malignant/non-malignant hematology.
j) Establishes and manages a pharmacy clinic to support on-going care of Veterans with cancer.
k) Monitors patients as part of the infusion center and aids in clinical decision making when needed for urgent patient care.
l) Record incidences of clinical impact on patient care wherein interaction with physicians and/or nurses has positively affected the patient's course of therapy.
m) Evaluation of non-formulary consults in accordance with VA criteria for use of non-formulary medications.
n) Monitors use of non-formulary medications and offers alternatives to medical staff.
o) Monitors outcomes of the oncology patient population.
Pharmacy Operations
a) Interprets medication orders from the oncology infusion clinic or other clinics providing oncology care for accuracy and appropriateness.
b) Reviews drug therapy selections for drug related problems including, but not limited to drug-drug interactions, duplicate therapy, contraindication, precautions, side effects, allergies, dosing, route, frequency, etc.
c) Identifies, resolves and communicates when prescribing or dispensing errors occur and documents interventions as necessary.
d) Collaborating with the Chief of Pharmacy and pharmacy operations team to ensure the safe preparation, handling, monitoring, and disposal of anti-cancer drugs as well as compliance with applicable VHA guidance.
e) Coordinating the preparation and dispensing of medications with the inpatient pharmacy and infusion center.
f) When necessary, prepare and dispense medications, including investigational medications, as requested per medication orders according to established policies, procedures and protocols.
g) Process, dispense and/or distribute controlled substances and maintains appropriate accountability and records as required by agency regulations.
h) Actively support adverse drug reaction and medication error reporting for the institution.
i) Provide prompt, accurate drug information when requested.
j) Evaluate use of medications in the infusion center medication room for appropriateness included the need to add/delete medications.
Work Schedule: 8:00 am - 4:30 pm Monday - Friday.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: To be qualified for this position at the GS 13 level, you must meet the following requirements:
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level
AND
You must demonstrate the following Knowledge, Skills, and Abilities (KSA):
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: PGY-2 Residency in Oncology or PGY-1 Residency with 2 years of Oncology experience.

The full performance level of this vacancy is GS-13.
Physical Requirements: Physical movements involve completion of routine pharmacy functions which include but are not limited to significant computer work/typing, prescription order entry and processing, contact of patients (both face to face and via phone). Certain tasks are repetitive in nature and require multiple steps performed in succession for success. The incumbent should be able to lift and carry up to 50 pounds without significant difficulty. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging.
