The Robert J. Dole VA Medical Center is seeking a highly qualified and motivated candidate for a Clinical Pharmacy Specialist - Acute Care position within Pharmacy in Wichita Kansas. Work Schedule: Monday - Friday, 7:30 AM - 4:00 PM with occasional weekends and holidays. Financial Disclosure Report: Required FLSAFLSA: Exempt Virtual: This is not a virtual position. Duties

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Acute Care Program Manager must be a licensed pharmacist and is accountable for providing leadership, direction, organization, planning, and implementation of pharmacy programs which promote stewardship, patient centered care, patient safety, and optimization of evidenced based pharmaceutical care outcomes. The incumbent is responsible for the development, implementation, monitoring, evaluation, and reporting of the facility's acute care pharmacy services and overseeing the acute care residency rotations at the Robert J Dole VA Medical Center (RJD). This position will serve as a residency preceptor and collaborate with the residency program director (RPD) to enhance the resident learning in acute care. This includes, but is not limited to, the following: a) Act as an independent provider operating under a scope of practice and providing expertise to other members of the multidisciplinary team(s). b) Conduct patient interviews to include, but not limited to, medical history, medication reconciliation, assessment, treatment planning and clinical outcomes. c) Make recommendations to change, delete or add to the current medication regimen as appropriate to optimize the cost/benefit ratio of the regimen within the guidelines of the P&T Committee, the Preferred Drug Program, treatment protocols, and our network formulary. Notes all changes in the patient's medical record. d) Renew prescriptions for up to 12 months to continue medication therapy in stable patients when appropriate. e) Discontinue medications that are no longer needed by the patient, that are an unnecessary duplication of therapy, or that are causing unwanted side effects. Communicates changes to the patient's provider appropriately f) Document all patient interactions and assessments in the medical record promptly and accurately g) Ensure safe, appropriate, cost-effective medication therapies for patients, in accordance with established policies, procedures, and protocols. h) Communicate regularly with the Veteran's provider concerning the ongoing medication therapy management i) Initiate and adjust medication dosages of prescribed medications in response to lab values, drug levels, and patient status within scope of practice j) Interact directly with patients by evaluating each patient's ability to understand medication instruction and providing oral and written counseling as appropriate on his/her disease state and drug therapy. k) Process inpatient non-formulary and non-preferred drug requests through in-depth evaluation of the patient's condition and recommends alternatives when indicated. l) Implement and monitor formulary management protocols and programs approved by the Pharmacy & Therapeutics Committee. Utilizes these drug management tools in a manner that supports the drug budget goals of the health care system. m) Interfaces with physicians and other health care professionals in the care of patients for optimization of rational safe, appropriate, and cost-effective drug therapy., with special emphasis on the use of anti-infectives. The Acute Care Clinical Pharmacist Specialist, as a member of the pharmacy staff, functions under the supervision of the Associate Chief of Operations and the Chief of Pharmacy. All assigned program management and clinical activities shall be completed in accordance with approved clinical pharmacy protocols.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (TRANSCRIPT REQUIRED) Licensure: Possess a full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the U.S. (i.e. Puerto Rico), or the District of Columbia. NOTE: Required before final job offer can be extended. Language: Be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Grade Determinations: GS 13 Experience: One-year of experience equivalent to the next lower grade level (GS-12). Equivalent experience to the next lower grade level (GS-12) may include, but are not limited to: Reviewing, interpreting, and verifying medication orders for appropriateness. Processing and filling medication orders, interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care. Reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems. Contacting providers as appropriate, documenting recommendations and interventions, providing refill extensions and partial medication supplies. Taking health and medication histories, performing medication reconciliation, providing drug information. Assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations. Documenting and assessing adverse drug events (ADEs), assisting in medical emergencies, providing oversight of technical staff in all aspects of medication distribution. AND You must demonstrate the following Knowledge, Skills, and Abilities (KSA's): Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. **You must address the Knowledge, Skills, and Abilities in your resume' for us to be able to best qualify you** Preferred Experience: CPS candidates who have completed an American Society of Health-System Pharmacists (ASHP) accredited specialty residency in critical care or general pharmacy practice, and/or obtained a board certification in pharmacy practice (i.e., BCCCP, BCIDP, BCPS, BCGP, etc.). Board Certification must be obtained through a national organization in which the quality of the certification is recognized by the Clinical Pharmacy Scope of Practice Review Board and the Pharmacy Practice Standards Board. Physical Requirements: Physical movements involve completion of routine pharmacy functions which include but are not limited to significant computer work/typing, prescription order entry and processing, contact of patients (both face to face and via phone). Certain tasks are repetitive in nature and require multiple steps performed in succession for success. The incumbent should be able to lift and carry up to 30 pounds without significant difficulty. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging.
