The incumbent will provide direct patient care and work primarily as a telework provider in support of VCB CBOCs clinical team's contingency and transitions of care needs. They may also function, as needed, as a member of an expanded healthcare team (high-risk department, social work, transition specialty clinical pharmacist, and PACT care manager) to coordinate transitions of medical care for a population of Veterans from community based inpatient services to a group practice setting.

Duties

Major Duties of this position include, but are not limited to: 1. Coordinate care concerning placement after acute inpatient hospitalization discharge i.e., IRU, LTAC, home, SNF, outpatient hospice, etc. 2. Coordinate care concerning need for Home Health Orders (GEC/LIP Notes) i.e., wound care, parenteral antibiotic, speech therapy, physical therapy, etc. after discharge. 3. Coordinate care concerning the required durable medical equipment (DME) for a safe discharge i.e., home oxygen, walker, hip kit, etc. 4. Coordinate care concerning specialty consults required after discharge i.e., community care infectious disease (ID) when in house service is not available. 5. Coordinate care with extended service lines (nursing, social work, and clinical pharmacist), updating active problem lists, and active medication list prior to primary care provider post hospital face to face visit(s). 6. Coordinate care alongside high-risk department to expedite order and consult entry after discharge of high-risk veterans and presence in the transition of care weekly meetings with community hospitals. 7. Prescribe medications within defined scope of practice. 8. Order diagnostic studies such as laboratory tests, x-rays, electrocardiograms 9. Provide education and counseling of patients and families in preventive care, medical conditions, and use of prescribed treatments and drugs when required. In addition, Transitions of Care Nurse Practitioners: o Understand and maintain fidelity to the VHA mission. o Embody and demonstrate I CARE values in all interactions with Veterans, family members, staff and community stakeholders. o Are responsible for compliance with all applicable VHA, VISN and VATVCBHCS policies, rules and regulations as well as adherence to the VATVCBHCS Medical Staff Bylaws. o Provide leadership in the application of the PACT model and clinical practice guidelines to organizational processes and/or systems in order to optimize patient care outcomes, achieve excellence in both internal and external customer service and shape collaborative, high-performing teams. o Complete PACT 100 training as well as all assigned TMS and other mandatory training. o Complete telehealth training and maintain capability of delivering telemedicine services to Veterans across the VATVCBHCS as needed. o May serve on committees and quality improvement projects as assigned. o May participate in teaching/supervision of students and residents. In which case, compliance with GME requirements while fostering an environment supportive of education, training and research is required. o Participate in the peer review process as assigned. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Nurse Practitioners and Clinical Nurse Specialists. On and after [March 17, 2009], registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before [March 17, 2009], who obtain such qualifications on or after [March 17, 2009.] Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Education
