This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application

Duties

Coordinating the day-to-day transitions of Veterans who will be discharged from the substance use disorder (SUD) DOM to less restrictive programs. Consulting with other professionals at the medical center and community organizations to develop a clear understanding of the social, economic, medical, and psychiatric factors that impact the continuing care needs of Veterans referred. This includes contact with any stakeholders (e.g., veterans, families, community hospitals, nursing homes, Veterans Service Organizations, etc.). Coordinates the discharges and transitions of veterans according to VHA Directives. Participates in professional staff development activities. Develops data driven performance improvement monitors with established goals and outcomes to evaluate and document the effectiveness of the discharge process. Screening /Assessment of Veteran of Veterans with Substance Use Disorders and co­occurring psychiatric disorders. Treatment Planning/Goal Setting -Incumbent is responsible for developing the treatment plan and setting achievable treatment goals with the resident. Psychotherapy- The social worker will provide evidence-based treatment modalities. Selects and provides the most appropriate psychotherapeutic techniques in providing quality care, including evidence-based care for Substance Use and co-occurring disorders. Psychoeducation -The incumbent will provide education on topics including illness management, relapse prevention and recovery groups. Crisis Intervention -The social worker is experienced in making rapid assessments and developing crisis management plans for admission to mental health inpatient programs. Advocacy -The clinical social worker understands the intimidation of bureaucracy and will act as an advocate when it serves the best interest of the veteran/family. When appropriate and feasible, social worker will educate and encourage the veteran/family to advocate on their own behalf, thus fostering a sense of independence and empowerment. Qualifications

To qualify for this position, applicants must meet all requirements within 15 days of the closing date of this announcement. Basic Requirements: U.S. Citizenship: Must a citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. Education: Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure. Must be licensed or certified by a state to independently practice social work at the advanced practice level to qualify at the GS-12 level. (COPY OF LICENSE REQUIRED) IN ADDITION TO THE BASIC REQUIREMENTS ABOVE, YOU MUST MEET THE FOLLOWING GS-12 GRADE REQUIREMENTS: Experience: Must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget. Applicants must also meet the Demonstrated Knowledge, Skills, and Abilities listed below: 1. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program. 2. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring. 3. Ability to supervise multidisciplinary staff assigned to the program. 4. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s). 5. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice. References: VA Handbook 5005/23, Part II, Appendix G39. Physical Requirements: The work is sedentary, but requires some walking, standing, bending, the carrying of light items such as books and papers, and traveling.
