The part-time (0.5) Clinical Pharmacist Specialist position under the Pharmacy Service at the Charles George VA Medical Center, Asheville NC, provides comprehensive medication management and clinical pharmacy services through direct patient care in Primary Care.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Initiates and provides clinical pharmacotherapy services in ambulatory care clinics and other clinics as designated by the Clinical Chief, Pharmacy Service and Chief, Primary Care. Provides advanced pharmacology and therapeutics consultation to the Medical Center professional staff regarding ambulatory care pharmacotherapies. Evaluates non-formulary drug use through in-depth evaluation of patient condition and recommends alternatives when indicated. Obtains medication histories from patients and summarizes significant findings. These findings are documented in progress notes in the electronic medical record and are communicated to the patient care team. Identifies actual and potential drug-related problems that may affect ambulatory care patients and acts to achieve problem resolution. Works collaboratively with clinical pharmacist specialists and patient care team in an effort to achieve continuity of care. Weighs the risk: benefit ratio of various therapeutic alternatives, develops a therapeutic plan utilizing the cost-effective pharmacologic treatment modalities available and institutes the plan or communicates the plan to the patient's primary care provider(s). Develops individualized dosing regimens based on the pharmacokinetics and pharmacodynamics of the drug(s) employed and patient condition and institutes the regimen or makes appropriate recommendations to the primary care provider(s). Develops rational alternative treatment plans in cases where the initial plan fails to achieve the desired therapeutic result and communicates this this with the primary care providers. Educates patients on the proper use and care of their medications and medication refill procedures. Evaluates and ensures optimal medication regimens through analysis of medication profiles, laboratory data, medical and social data and direct patient observation or interaction. Assists in the collection, reporting and assessment of Adverse Drug Reactions. Applies drug literature analysis to develop and design clinically oriented responses to anticoagulation and primary care staff. Provides literature interpretation and recommendations to the Pharmacy and Therapeutics Committee as a consultant in drug information. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grandfathering Provisions: All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. Note: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard 5005/55 Part II, Appendix G15 for this occupation; (only applicable to current VHA employees who are in this occupation and meet the criteria. Grade Determinations: GS-13-In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. 1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. 2. Expert understanding of regulatory and quality standards for their program area. 3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. 4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. 5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
