This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Social Worker Program Coordinator acts as the Admission/Discharge Coordinator for Veterans transitioning to/from the MH RRTP. The incumbent must be licensed by a state at the independent practice level. The incumbent will adhere to the NASW Code of Ethics, all local social work policies, and maintain an unrestricted, unsuspended license to practice social work. Description: As Admission/Discharge Coordinator, the incumbent evaluates Veterans' appropriateness for admission, manages a waitlist, acts as a liaison to referring clinicians, and coordinates the admissions process. Must use a high level of skill in assessing and coordinating care for the complex psychosocial problems of Veterans as they transition from the MH RRTP to appropriate discharge alternatives. The incumbent supports the MH RRTP RN Case Managers in performing transitional care management functions for current, future, and recent residents. Duties: The incumbent is able to provide professional, independent social work services to Veterans who are experiencing issues related to Post Traumatic Stress Disorder and Substance Use Disorders and who are experiencing a wide range of complex medical, psychiatric, emotional, behavioral, and psychosocial problems. Assesses and documents identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence Acts as an advocate with appropriate VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregiver. Independently assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. In collaboration with the Veteran, family/caregiver, and interdisciplinary treatment team, the incumbent facilitates the delivery of health care services. Actively participates as a member of the interdisciplinary treatment team and admissions team. Collaborates with Veterans, family, and interdisciplinary treatment team members in the development and implementation of a resident centered treatment plan. Provides case management and care coordination to facilitate appropriate delivery of health care services and incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Links the Veteran with services, resources, and opportunities, in order to maximize the Veteran's independence, health, and well-being. Conducts timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. Provides a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy. Implements a treatment plan, in conjunction with the interdisciplinary team, including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity. Serves on committees, work groups, and task forces at the facility and VISN level or in the community, as assigned by the supervisor. Provides subject matter consultation to colleagues and students on the psychosocial treatment of Veterans, offering professional opinions based on experience, expertise and role modeling effective social work practice skills. Establishes and maintains ongoing education programs for Veterans, community agencies, students, and staff, to facilitate understanding of social work interventions specific to the Veteran/Military population. Facilitates discharge planning through collaboration with Veterans and their families, as well as interdisciplinary treatment team members, to ensure that appropriate discharge plans are executed in a timely manner. Provides consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives. Provides ongoing documentation of Veteran's treatment progress using a psychosocial and behavioral framework. Attends Mental Health Service Line, Social Work Discipline meetings, and other mandatory meetings regularly. Maintains current BLS and/or First Aid certification and remains current on all licensures. The incumbent performs other related duties as assigned. The Cheyenne VA Health Care System, Mental Health Service Line is currently recruiting for (1) Social Worker (Program Coordinator) to be assigned to The Mental Health Residential Rehabilitation Treatment Program (MH RRTP). DUTY LOCATION (s): This position is located at Loveland, CO, but does require occasional face-to-face work working at the Cheyenne main facility. This position will require regular travel, requiring operation of a government vehicle. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/ Grandfathering Provision: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS 12 Social Worker Program Coordinator Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is a GS-12 Physical Requirements: Typically, the employee sits comfortably indoors to do the work. This position requires potentially long periods of continued walking; standing; sitting, bending; carrying of light items such as papers, books, small parts; or driving an automobile; etc. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16", hearing (aid permitted), clear distinctive speech.
