Cheyenne VA Medical Center is hiring for one Housing and Urban Development-VA Supportive Housing (HUD-VASH) Geriatric Extended Care (GEC) Senior Social Worker. This position is located in Loveland, Colorado, and may be eligible for telework within the local commuting area based upon supervisor and facility approval. The incumbent will be required to travel regularly within the CVAHCS catchment area, requiring operation of a government vehicle.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Housing and Urban Development-VA Supportive Housing (HUD-VASH) Geriatric Extended Care (GEC) Senior Social Worker is assigned to the Mental Health Service Line (MHSL) in the HUD-VASH Program and will work autonomously providing high quality, focused clinical care and case management support to aging Veterans enrolled in VA housing services. The incumbent performs social work duties specifically directed toward homeless Veterans and their families or Veterans at risk for homelessness and functions as an advanced practice Social Worker. The incumbent is responsible for providing case management services to Veterans eligible and approved for the HUD-VASH housing voucher; providing case management services necessary to ensure that Veteran care is coordinated with the Veterans Affairs Health Care System (VAHCS), community health or mental health providers, and agencies that serve homeless and aging/disabled populations; and serving as the primary contact with local Public Housing Agencies (PHAs). The incumbent will also serve as the liaison between HUD-VASH and VA GEC services to support collaboration between programs to maximize independent living, along with building relationships with community agencies servings aging individuals. Other duties may include but are not limited to: The GEC HUD-VASH Senior Social Worker is a direct care provider and manages a caseload of HUD-VASH Veterans, along with a subset of aging Veterans using a high level of skill in assessing and treating, within scope of licensure, the complex biopsychosocial problems of homeless Veterans and their families/significant others, while integrating specific knowledge of social determinants impacting aging populations. Maintains appropriate and timely records, to include HOMES database entries, Opening Notes, Initial Psychosocial Intakes and History, treatment plans, progress notes of each encounter, termination notes, and other required documentation. Independently formulates and implements a treatment plan in collaboration with the Veteran and families/caregivers when appropriate. Adjusts the psychosocial treatment plan and interventions based on changing needs and response to interventions. In cases of antagonistic, indifferent, or evasive Veterans and their significant others, the Social Worker deals constructively with both positive and negative reactions, understands disparate values, and reaches Veterans and families from different social and ethnic groups. Communicates confidence in the Veteran's ability and worth, helping the Veteran address problems utilizing larger support systems as appropriate. Makes recommendations for care and changes in interdisciplinary treatment plan to include level of case management and/or graduation. Engages Veterans in exploring higher level of care interventions when necessary and making appropriate referrals. Supports and assists supervisors in strategic planning related to the implementation of GEC services, provision of groups programming, and the utilization non-VA GEC programming where indicated. Supports the provision and implementation of clinical supervision and training to interns, fellows, and students at the programmatic level for Homeless/HUD-VASH services. This is done in close coordination with direct supervisors, discipline executives, and training directors. Under the direction of the HUD-VASH Supervisor, the incumbent may assist in planning, establishing, and implementing policies and procedures related to GEC services. The incumbent may assist the HUD-VASH Supervisor with overseeing the data collection and administrative record keeping, as well as monitor patient or program clinical indicators, thresholds, and performance measurements for HUD-VASH and GEC tracking. Serves as the primary point of contact representing the HUD-VASH team with GEC National Clinical calls and disseminating information to the team for best practices. Provides crisis intervention when indicated including evaluation, assessment, triage, initiation of and support in Emergency Detention procedures within scope of licensure, and disposition of patients experiencing behavioral health emergencies. May provide services using Tele-Mental/Tele-Home technology. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determination GS-12 Senior Social Worker Licensure/Certification. Individuals assigned as Senior Social Workers must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Education and Experience. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. This position is being hired above the full performance level at GS-12. This position requires potentially long periods of continued walking; standing; sitting, bending; carrying of light items such as papers, books, small parts; or driving an automobile; etc. Use of fingers, both hands required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16", hearing (aid permitted), clear distinctive speech. The incumbent must be able to drive an automobile.
