This position is located at the Brooklyn VA Medical Center, New York. The incumbent is positioned under Primary Care. The incumbent ensures that data input in primary care systems is accurate to properly determine the future needs of staff, PACT teams, and capacity.
Duties
The identifications of PACT, their associated FTE, and space used as well as the management of patient panels through the mandatory and consistent use of primary care systems, allows facilities to track patients and their associated PACT team and patients.
Duties:
DATA ENTRY AND DATABASE MAINTENANCE
- Careful data collection, monitoring and maintaining provider profiles, monitoring panel sizes, oversight and collection of transmission errors, and review/rectification of incorrect patient assignments.
- Prepares National, VISN, and Medical Center level reports, upon request using established computer programming routines.
- Collects, compiles, analyzes, and presents data regarding the integrity of the PCMM package.
- Assure training, implementation; monitoring, and evaluation of all administrative procedures related to PCMM operations and is the facility resource on all Issues that relate to the Patient Care Management Module and the management of this critical database.
- Serve in an administrative capacity to executive management in the service.
- Enrolls patients and selects the appropriate provider with the PCMM system.
- Assists in the preparation of reports and briefings on the results of studies and is responsible for the collection, interpretation, and analysis of quantitative and qualitative data relevant to programs being evaluated utilizing the PCMM program.
DATA ANALYSIS
- Is a primary data managing and recommending resource for Primary Care Leadership when mission-critical updates/problems concerning PCMM are identified at the facility, regional or National levels; and serves as a subject matter expert for new technology assessment and usability related to PCMM goals, objectives, implementation, and sustainment.
- Uses data to influence healthcare systems to drive the management of the health system toward effectiveness and efficiency within facility clinic areas to improve and sustain operations and access to care.
- Routine reports regarding workload including panel size, team assignments, enrollment capacity, available capacity, etc. are generated on a daily, monthly, and quarterly basis.
PROJECT MANAGEMENT AND REPORTING
- Assists the Primary Care Chief in projects by developing custom reports needed to develop an in-depth analysis of all Primary Care.
- Is knowledgeable and participates in the prioritization of resources.
- Runs reports of workload and ensures deviations from the planned workload are detected and addressed.
PCMM STATION EXPERT
- As the data analysis expert provides relevant clinical and /or administrative data to identify and resolve organizational problems.
- Consults with IT to integrate software applications appropriately and for data reporting methods for clinical services
MANAGEMENT OF PROGRAM MANAGEMENT FUNCTIONS
- Manages administrative functions associated with the PCMM for the facility.
- Assists professional staff with and handles administrative functions of the office.
Performs other duties as assigned.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/27/2023.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement.
For a GS-07 position you must have served 52 weeks at the GS-05.
For a GS-09 position you must have served 52 weeks at the GS-07.
The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. You may qualify based on your experience and/or education as described below:
GS-07 Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 grade level in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: federal service and that is typically in or related to the work of this position. Specialized experience includes: with more specific instruction and guidance from close supervision: Responsible for organizing, planning, meeting deadlines, maintaining excellent records, and following through daily, periodic, and situational needs. Applies a wide range of methods used to gather, analyze, and evaluate information concerning management processes, draw conclusions based on the information and provide recommend appropriate actions, prioritize projects, and manage action items. Communicates effectively with managers and employees at all levels. Excellent communication skills, both written and oral, to prepare memoranda, letters, policies, procedures, and a variety of reports. NOTE: Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Education: Applicants may substitute education for the required experience. GS-09 Specialized experience: - One (1) full year of specialized experience equivalent to the GS-7 level in the Federal service and that is typically in or related to the work of this position. Specialized experience includes: Advanced working knowledge of primary care healthcare delivery processes, the mission and vision, and core values of VHA, Medical Center policies and procedures, and VHA directives and regulations as they relate to primary care functions. Creating and updating various reports which require the incumbent to apply analytical and evaluative methods and techniques. Working with detailed knowledge of outpatient processing, medical record creation, content and format, workload data capture, encounter forms, scheduling, electronic wait lists, and primary care systems and applications. Responsible to build and submit statistics, analytic proposed projects, reports, and recommendations of data to effectively convey accurate information to the Service Chief, Medical Center, VISN, or national program manager.-OR-
Education : Have successfully completed a master's or equivalent graduate degree, or 2 full years of progressively higher level graduate education leading to such a degree, or an LL.B. or J.D. in a related field that demonstrates the knowledge, skills, and abilities necessary to do the work of this position. Combination: Possess equivalent combinations of successfully completed graduate level education (in excess of the first year or 18 semester hours) and specialized experience to meet total experience requirements. ???????Travel to facility CBOCs and divisions/sites of care is required to verify/validate rooms, staffing, and primary care setup on a periodic basis. The PCMM coordinator is responsible for PACT staffing, patient capacity, panel size, and exam room space per VHA Directive 1406.
