The primary role of this position is that of a Patient Care Coordinator/Case Manager who will be responsible for coordinating patient care activities for the Emergency Department & Outpatient Walk-In Clinic as to promote comprehensive patient-centered care. The incumbent must be able to assess & prioritize each patient's requirements based on their specific-age needs. Routine nursing duties may include the following areas; educational systems, patient homes, other clinics & job sites as directed
Duties
Directly supports patients, providers and clinic staff.
Provides case management for patients by coordinating services to meet patients care goals.
Pro-actively supports and is the designated lead & point of contact for the Patient Centered Medical Home and SD State Health Home Initiative for the Service Unit.
Conducts utilization review meetings when idenfitifed as needed.

Education
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience
The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position.
Basic Requirements for the GS-11 Grade Level
In addition to the above requirements, applicants must meet one of the following requirements:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently planning and providing nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals and providing oversight to ensure compliance and appropriateness of care OR successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
