The VA Greater Los Angeles Healthcare System (VAGLAHS) is seeking an experienced Registered Nurse to work as Patient Safety Manager (PSM) in our Quality Management Department. The PSM is responsible for identifying patient safety risks, establishes internal controls, implements resolutions, and measures outcomes with respect to all services and ensures that Veteran-centered healthcare is provided and delivered in manner consistent with the mission, goals and objectives of the organization.

Job duties include, but are not limited to, the following:
Advanced knowledge of Quality Management/Performance Improvement principles, methodology and processes including use of a variety of statistical tools.
Advanced knowledge of patient safety principles and the mitigation of risk that lead to innovative, evidence-based approaches in the improvement of patient care delivery.
Advanced knowledge/experience with healthcare accrediting organizations as it relates to patient safety and risk (e.g. The Joint Commission) and regulatory government agencies.

Promotes a culture of safety within the organization and partners with clinical and administrative support staff to design, modify, and implement systems that assure patient safety goals are met and safety processes are improved.
Act as a change agent and mentor to staff regarding Patient Safety.
Provides consultation to leadership and staff to assure continuous improvement and progress toward improving patient safety and ethical resolution of concerns.
Coordinates and evaluates service lines and/or integrated programs to ensure safe patient care is administered by all professional and non-professional personnel according to organization and regulatory policies and guidelines.
Designs and delivers Patient Safety training/education events, serving as a staff resource within GLA, and partners with other services to embed patient safety approaches in department/unit activities and processes.
Contributes to the professional growth and development of colleagues and other health care providers by providing opportunities to engage in Patient Safety projects, research activities, and professional publishing.
Provides useable, relevant data to Executive Leadership regarding results, issues, and progress in patient safety and risk mitigation.
Applies rigorous analytical, systematic, data driven continuous improvement approaches to process and system redesign.
Manages and responds to alerts/advisories from the National Center for Patient Safety Data Base, including timely notification, resolution and documentation of same. Oversees Root Cause Analysis identification, investigation, actions, and follow-through (including reviewing situations, assigning Safety Assessment Codes scores and leading/facilitating root cause analysis teams).
Conducts Healthcare Failure Mode Effect Analysis/Proactive Risk Assessment.
Actively represents GLA Patient Safety and collaborates with various national, state and local committees, councils and workgroups.
Actively represents GLA Patient Safety with all visiting certifying/accreditation agencies.
Oversees all GLA Patient Safety initiatives, including any newsletters, flyers, brochures etc. Leads/facilitates improvement teams as requested, developing productive working relationships with groups, programs and/or services.
Prepares an annual report detailing Patient Safety activities and progress.
Delivers professional presentations at national, state, and local forums to highlight GLA Patient Safety Programs.
Manages and utilizes a variety of automated/software systems and supporting processes for Patient Safety including but not limited to: VISTA, Joint Patient Reporting System, Patient Safety Assessment Tool, the National Patient Safety Center SPOT and Alert/Advisory programs, Computerized Patient Record System, and VISTA extracted reports.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
For foreign education equivalency the only certifying body the VA recognizes is CGFNS and its' subsidiary, International Consultants of Delaware (ICD). Without certification of BSN-equivalency, the VA can only consider the degree at the associates level and pay will be determined accordingly.

Physical Requirements: Heavy lifting (45 lbs and over); Heavy carrying (45 lbs and over); Straight pulling and pushing; reaching above shoulder; use of fingers; both hands required; walking and standing up to 8 hours; repeated bending; Ability for rapid mental and muscular coordination simultaneously; hearing aid permitted.
