Duties
The Suicide Prevention Coordinator (SPC) is an advanced practice social worker assigned to one location in the Salisbury VA Health Care System. The incumbent's primary responsibility is to provide evidence based clinical services (assessment, treatment planning and therapy), program development, Veteran and staff education; outreach; continuous quality improvement activities; and evaluation/consultation. In addition, the incumbent must be a skilled communicator capable of developing effective partnerships with internal and external customers. The incumbent is administratively responsible for the clinical programming and daily operations at the program assigned. The incumbent develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. The incumbent manages and controls the use of data related to the program regarding day-to-day functioning and program evaluation.
Duties include but are not limited to:
Program Management (approximately 50% of time)
The overall implementation and function of the Suicide Prevention Program at the assigned location.
Develops and implements the Suicide Prevention Program at their location and ensures compliance and collaboration with local facility, VISN and VA Central Office initiatives as appropriate for after Suicide Prevention.
Assesses the needs of the population served to determine treatment needs and programs to be developed.
Develops programming to address the needs of the Veteran population considering local needs, VISN needs and VA direction such as the Uniform Services Handbook.
Coordinates ongoing evaluation to ensure effective treatment using best practices.
Identifies unmet needs and works collaboratively with facility, VISN and VA Central Office staff to develop and implement solutions.
Administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans and families in coordination with the overall goals of the medical center and those of the VISN and VA Central Office.
Develops policy and procedures to ensure compliance with the Joint Commission, Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards as needed.
Provides training to staff regarding services offered in the Suicide Prevention program. Coordinates and oversees the Suicide Prevention Education at their location.
The SPC prepares, plans and advises the Suicide Prevention Program manager of anticipated changes in workload, staffing needs, and program developments. The incumbent monitors clinical aspects of the SPP for coverage as needed. The coordinator provides consultation, on request, to other Suicide Prevention staff and interdisciplinary team members on complex, difficult cases, using advanced practice skills and expertise.
Oversight of Care Coordination (approximately 50%)
Oversees the Suicide Prevention Case Manager(s) (SPCM), including monitoring their caseload and the advocacy services provided to high risk for suicide Veterans in their caseload(s).
Provides follow up on consults (within 24 business hours) received from the Veterans Crisis Line to assure timely access to care and follow-up for patients in crisis. When needed, a member of the SPT assists with and expedites Veteran enrollment into the VA.
Maintains a reduced caseload. Along with the Suicide Prevention Case Manager(s) ensures contact is made with Veterans who have been identified as surviving an attempt or who are identified as being high risk. Personal contact will be made with the Veteran and the SPT will establish US mail contact with the Veteran in order to provide an "enhanced level of care" model of prevention which maintains communication with the Veteran.
Along with the Suicide Prevention Case Manager(s), assures that all Veterans on the facility high risk for suicide list are being followed closely by the appropriate provider on a regular basis as determined by the coordinator and the provider.
Along with the Suicide Prevention Case Manager(s) tracks any no-show appointments by these Veterans and following up with the Veteran as soon as possible to reschedule the appointment.
Ensures all Veterans identified as high risk for suicide is afforded the opportunity to complete a Safety Plan that is documented in CPRS and that the Veteran is provided with a copy of their Safety Plan.
Ensures that patients identified as being at high risk for suicide receive follow-up for any missed mental health and substance abuse appointments in conjunction with the clinical treatment team, and that this follow-up is documented in the electronic medical record.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).

Grade Determinations:
Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.

The full performance level of this vacancy is GS12.
Physical Requirements: Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and standing, bending and carrying such items as books, papers, and files. In carrying out responsibilities, it may be necessary to travel into the community where he/she conducts interviews with the Veterans, their families, representative of community health and welfare agencies and law enforcement agencies. Must possess current driver's license and be able to drive a government vehicle in carrying out professional duties when deemed neccessary.
