The Home Based Primary Care (HBPC)/Non-Institutional Care (NIC) Department has one opening for a Program Director who will be aligned under the Chief of Geriatrics and Extended Care (G&EC). This incumbent will provide executive leadership and assume continuing and substantial responsibility for the coordination and evaluation of an integrated program that crosses service and/or discipline lines and influences organizational mission, vision, values, strategic priorities, and clinical outcomes.

Duties

DUTIES: The Home Based Primary Care (HBPC)/Non-Institutional Care (NIC) Program Director primary duties will include, but are not limited to: The incumbent facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between services where staff are matrixed to HBPC and all healthcare disciplines included on the HBPC teams. The HBPC/NIC Program Director is responsible for directing the clinical and administrative services offered by the programs to ensure the programs are in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations. Responsible for the development and continued effective functioning of the interdisciplinary health care team, recognizing and utilizing the skills, knowledge and contributions of each team member. Collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis. Responsible for the interpretation and communication of VA policies to the HBPC and/or NIC teams and the Charles George VA Medical Center. In addition, he/she is responsible for communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary. Responsible for the HBPC team development and implementation of local policies and procedures to include performance improvement, patient safety, emergency preparedness, and staff safety. Responsible for the evaluation of the effectiveness of the HBPC and NIC programs and assuring effective/efficient resources are maintained. He/she assures the programs are able to optimize cost effectiveness (budget) and patient outcomes. He/she assesses the continuing education needs of program staff and assures appropriate resources to meet those needs. Responsible for managing and tracking program resources to include but not limited to equipment inventory, GSA and VA owned Vehicles. He/she oversees the processes for assigning and tracking referrals for HBPC and the NIC program and ensures timely access. He/she collaborates with Geriatrics and Extended Care leadership, the Facility Leadership and Fiscal Service to ensure accurate reporting of HBPC and NIC clinical, administrative and workload data. He/she has the major responsibility for implementation of the vision, mission, philosophy, and core values for the organization. The incumbent is responsible for the management and oversight of the HBPC and NIC programs. The incumbent contributes to Medical Center policy development, planning, decision making, resource management and evaluation that affect the delivery of health care services to patients. Facilitates the approval for the referral process to the HBPC program and other VA staffed in-home programs. He/she supervises administrative and professional clinical staff within the HBPC program and other VA staffed in-home programs. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Leadership and Home Care Management Strongly preferred Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience. Nurse V - Master's degree in nursing or related field with BSN (or a Bachelor's degree in a related field) and approximately 5-6 years of nursing practice/experience; OR a Doctoral degree (preferred) in nursing or related field and appropriate basic or advanced certification desired, and approximately 4-5 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: In accordance with VA Handbook 5019, Part II, Paragraph 3, II-1, 1(a), A pre-placement physical examination to determine the physical, cognitive and emotional fitness of candidates for appointment in VA is required, and shall be completed prior to appointment, for all full-time individuals appointed under 38 U.S.C., Chapter 73, for Title 38
