THIS IS A DETAIL POSITION, NTE 120 DAYS (You must have manager approval for this detail position) - The incumbent of this position is located in Informatics, and has primary responsibility as the Patient Centered Management Module (PCMM) Coordinator for all sites for the VA Salt Lake City Health Care System (VASLCHCS) in conjunction with the Principal Facility PCMM Coordinator, and will provide high-level operational support and management expertise of the PCMM program.

Duties - Provide high-level operational support and management expertise of the PCMM program. Has responsibility to engage in developing, analyzing, evaluating, and advising on and/or improving the effectiveness of methods and procedures, organization, staff utilization management controls, information and documentation systems or similar functions of management. Has responsibility to engage in panel management and efficiency of work, research and data analysis work, and training programs related to Primary Care and Patient Care Service lines. Will be responsible for the integrity of VASLCHCS large PCMM Web database which integrates data and information from all divisions and VA community clinics. Required to work closely with VASLCHCS clinic support staff, clinical service chiefs, VA community clinic staff, MCA staff, VISN staff and Central Office staff to maintain integrity of this program and data. Develops designs and completes reports related to PCMM Web, PC and MH. Identifies and determines training priorities as they relate to PCMM Web, PACT or any of other areas of responsibility. Assists in performing organizational and/or manpower unitization studies of the clerical and technical aspects of the PC PACT clinics with related functions to identify and develop improvements and efficiency of manpower and methods to present to leadership. Helps in the development of new local policy and reviews established local policy in accordance with VHA Directive and delegations of authority which express the operating policies of the activity. Assists the MyVA Access Facility Champion in their responsibilities to examine access, workflow in the PC and MH clinics, to identify best practices and barriers to optimal care delivery. The PCMM Coordinator is responsible for: Assisting operating officials in implementing identified improvement actions and plans through participation in organizational improvement work through station programs such as Systems Redesign projects. Completing comparative analysis of VSSC reports or data sets with local PCMM data to identify data variance and ensure data integrity. Completing multiple assignment/reassignment to move large groups of PACT populations to different teams, when changes become necessary. Ensuring the integrity of the VA medical facility's PCMM application, its functionality, utilization, and resulting data. Ensuring that PCMM users are knowledgeable in PCMM functionality and utilization. Ensuring that PCMM PACT pending assignments are entered prior to the patient's first PACT appointment and no later than 24 hours from the creation of the patient's first PACT appointment. Ensuring accuracy of PCMM Web through analysis and evaluation to ensure compliance with VHA PCMM policies, guidelines, and business rules throughout the facility. Work with PFC to ensure that PCDPC data is entered for each PACT teamlet member in PCMM and entered as a portion of an FTE employee and is kept current and accurate. Work with PFC to establish and manage PCMM Web position set up; staff and FTE assignment, as well as patient assignments for all PC locations. Specifically, the incumbent, in conjunction with the PFC, will electronically create or modify team and position settings, assign staff to positions, assign FTE to positions, identify set maximum panel capacity, program clinical notification levels, and establish preceptor to associate provider links. Managing automatic inactivation's to ensure appropriateness and communication to PACT members. Meeting regularly with PFC and clinic managers and group practice manager to review provider panel management data, including access for face-to-face and telephone care. Participating in monthly national calls with the national PCMM Coordinator to ensure alignment with national policy, receive updates, and share strong practices. Participating in PCMM training activities and in national and/or VISN PCMM conference calls and discussion as needed. Serve as a technical expert for PCMM Web with the ability to troubleshoot and resolve user issues. Performs other duties as assigned. THIS IS A DETAIL POSITION, NTE 120 DAYS - To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/27/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. - For a GS-07 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. - For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Knowledge of administrative regulations and operating procedures plus skill in applying factfinding and investigative techniques. Knowledge of established management principles, pertinent administrative regulations, and staffing guidelines plus skill in applying fact-finding and work measurement techniques to conduct position management studies of clerical, trades, technician, and administrative support positions within a single organizational component at the operating (installation) level. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for the GS-07 grade level you must have 1 full year of graduate level education or superior academic achievement. OR, Combination: Applicants may also combine education and experience to qualify at this level. Specialized Experience GS-09: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization - Qualifications may include: Responsibility to engage in developing, analyzing, evaluating, and advising on and/or improving the effectiveness of methods and procedures, organization, staff utilization management controls, information and documentation systems or similar functions of management. Knowledge of management principles, organizational theory, and techniques of analysis and evaluation, along with knowledge of standardized clinic operational practices and procedures to assist in studies of clinic work processes in various organizations to identify, analyze, and recommend solutions to problems in organizational structure, staffing, administrative procedures, work processes, or workload distribution. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for the GS-07 grade level you must have 1 full year of graduate level education or superior academic achievement. To qualify based on education for the GS-09 grade level you must have a master's or equivalent graduate degree or 2 full years of progressively higher level graduate education leading to such a degree or LL.B. or J.D., if related. OR, Combination: Applicants may also combine education and experience to qualify at this level. The following is an example of how education and experience may be combined. This is an example only, and is not all-inclusive. The position to be filled is a Quality Assurance Specialist, GS-1910-5. An applicant has 2 years of general experience and 45 semester hours of college that included 9 semester hours in related course work as described in the individual occupational requirements. The applicant meets 67 percent of the required experience and 38 percent of the required education. Therefore, the applicant exceeds 100 percent of the total requirement and is qualified for the position. The position to be filled is a Management Analyst, GS-343-9. An applicant has 6 months of specialized experience equivalent to GS-7 and 1 year of graduate level education. The applicant meets 50 percent of the required experience but none of the required education, since he or she does not have any graduate study beyond that which is required for GS-7. Therefore, the applicant meets only 50 percent of the total requirement and is not qualified for the position. (The applicant's first year of graduate study is not qualifying for GS-9.)
