This Clinical Pharmacy Practitioner (CPP) functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients within the Hines VA Hospital system.
The CPP functions as member of assigned Internal Medicine (IM) team and serves as a mid-level provider to independently initiate, modify or discontinue medication therapy and as a consultant for medication therapy management services.

Major duties of this position will include, but are not limited to:
Clinical-Direct Patient Care Activities (some require scope of practice):
Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers.
Orders, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy.
Follows-up with patients on lab or test results to discuss the plan of therapy including changes in medication therapy, referral to primary care, or specialty provider.
Performs and documents medication reconciliation upon admission, transfer and discharge from the assigned service and informs appropriate healthcare providers of any discrepancies noted.
Provides recommendations for discharge and acts as a liaison between inpatient and outpatient pharmacies.
Independently adjusts medication regimens including anticoagulation based upon, laboratory, pharmacokinetic and pharmacodynamic data as available.
Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required.
Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed.
Participates in weekly rounds with assigned/covering medical team.
Documents clinical interventions in CPRS in a timely and professional manner as appropriate.
Clinical-Indirect Patient Care Activities:
Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes.
Utilizes VAADERs Adverse Drug Reaction (ADR) reporting system to monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues.
Evaluate patient care, recognizing abnormalities, and make dose adjustment recommendations.
Develop, upgrade, and disseminate criteria for use of formulary and non-formulary and other items.
Develops and conducts medication usage evaluation in areas of needs from a cost and patient outcome perspective.
Listens to all customer/patient feedback, positive and negative, acts to resolve complaints within his or her control, and reports feedback to management (or supervisor) in a timely manner.
Medication dispensing and distribution:
Identifies, interprets, and resolves prescribing and dispensing issues, including pharmacotherapeutics and the formulary.
Assist in coordination of activities of assigned pharmacy technicians and pharmacy volunteers.
Education:
Serve as preceptor for students, PGY-1 and PGY-2 residents and maintain preceptor qualifications that aligns with American Society of Health-Systems Pharmacists (ASHP).
Publish and/or present results of pharmacy projects/research studies, papers, etc.
Participates in development and implementation of education, ensuring training, or in-servicing of any health care provider including but not limited to: physicians, residents, technicians, pharmacists, students, or nursing; is completed on time or as needed if required.
Ensures training, or in-servicing of health care providers, is completed on time.
Administration/Technical Support:
Work with health care providers to develop programs for improvement of drug use.
Utilizes clinical video telehealth (CVT) and VVC technology to facilitate patient access.
Manages effective quality assurance program.
Develops and conducts medication usage evaluations in areas of needs from cost and patient outcome perspective.
Manages hospital formulary and ensures appropriate utilization.
Reviews regulation/standards and makes recommendations.
Serves on and participates in committees and attends meetings as subject matter expert.
Work Schedule: Monday-Friday, 7:30am to 4:00pm with ability to work any day of the week, including rotating weekends and holidays as needed.
Telework: This position has on-site requirements, but telework may be available on Pharmacy Supervisory management approval.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Note: Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Assignment-Clinical Pharmacy Specialist, GS-13
To be qualified for this position at the GS- 13 level, you must meet the following requirements:
Experience: In addition to the GS-12 requirements, must have one (1) year of experience equivalent to the next lower grade level (GS-12), which is directly related to this position, which includes: Handling routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations; reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution.

Pharmacists assigned to this position must demonstrate the following Knowledge, Skills, and Abilities (KSAs):
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience:
PharmD degree with completion of accredited PGY-2 Internal Medicine Residency; or
Completion of an accredited PGY-1 residency; or
Equivalent experience in Internal Medicine demonstrating competency.

The full performance level of this vacancy is GS-13.
Physical Requirements: The incumbent must be physically and mentally able to efficiently perform the duties of this position. The work requires regular and recurring physical exertion, involving standing/sitting for prolonged periods of time, frequent twisting/turning, bending/stooping, and ability to perform lifting/carrying, pushing, or pulling different types of equipment such as cabinets, carts or supplies weighing at minimum 50 pounds through various terrains.
