Coordinator manages the daily operation of the program and provides direct Veteran-centric services in the program area. Responsibilities include the daily operation of the program, developing policies and procedures for program operation and preparing reports and statistics for facility, VISN, and national use. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. The senior social worker is capable of practicing at the advanced level and has specialized knowledge of practice typically related to dementias, chronic illnesses, and care for the older adult and caregiver. Incumbent should have at least 2 years in an area of specialized social work practice and licensed or hold certification in a state at the independent practice level. Duties include, but are not limited to the following: Responsible for maximizing psychosocial quality of life issues in order to assist veterans to remain in their home or in a less restrictive level of care other than a nursing home, and keep them out of the hospital as often as possible. The incumbent serves veterans who tend to have serious, frequent and severe crises, lack family or an adequate community support networks, be poor at self monitoring; frequently fails to comply with instructions and treatments; have significant deficits in coping skills and require continuing psychosocial and medical support. Provides administrative oversight of program and administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans. Develops and implements information systems to track service activities such as consults, visits, referrals, inpatient days, Veteran demographics, equipment utilization and caregiver & Veteran satisfaction. Develops, implements, and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Provides assessment, treatment, and planning for a complex population of Veterans which reflect the age-specific, psychosocial, and functional needs of the Veteran (e.g., spinal cord, geriatric, and mental health population). Demonstrates knowledge of VA and community resources to meet the complex psychosocial needs of this unique population of Veterans and to ensure they are referred to the least restrictive environment that meets the level of care needed. Performs a wide range of coordination and education services, both internally and externally, and provides technical assistance to local, community partners. In addition, the incumbent uses sound judgment in the interpretation of local, regional, state, and national trends and patterns; the application of guidelines; and the recommendation of program enhancements. oversees program development, recruitment, and development of the foster homes, recruitment and admission of veteran residents, case management, development of policies and care processes, and coordination with interdisciplinary home care treatment teams. Incumbent has knowledge of federal and state laws and regulations that pertain to the care of vulnerable adults in Medical Foster Homes and is responsible for using critical judgment and discernment when expanding the program and accepting additional applicants. Performs work related to the review, monitoring, analysis, planning, and assessment of the Medical Foster Home Program. Independently deals with crises and effectively uses his/her problem-solving skills to assist the Veteran, family, and staff. Monitors homes with unannounced visits at least once per month. In addition, incumbent communicates with entire multidisciplinary team regarding Veteran's contentment with home and care received in the home. Adheres to the NASW Code of Ethics, all local social work policies, and maintain an unrestricted, unsuspended license to practice social work. Must be able to provide supervision for licensure. Provides the opportunity for professional growth and ensure competency of social work services across the continuum, social workers may be asked to cross train in other Social Work Service areas when functions are aligned with the current position of the incumbent. Qualifications

Basic Requirements Citizenship: Citizen of the United States. Licensure: Candidate must be licensed or certified by a state at the advanced practice level, and must be able to provide supervision for licensure. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Preferred Experience: 1 year working with medically ill patients Grade Determinations: GS-12 Social Worker (Program Coordinator) Experience: In addition to the basic requirements, applicants must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS[1]12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Physical Requirements: The work environment involves everyday risks or discomforts that require normal safety precautions typical of such places as offices, training rooms, and libraries. The work area is adequately lighted, heated, and ventilated. The incumbent must also work outdoors in all types of weather. There may be occasional exposure to moderate risks or discomforts in storage areas or hazardous waste sites.The work is primarily sedentary, although some physical effort may be required, e.g., walking, standing, climbing ladders, stooping, kneeling, and carrying light items such as manuals or briefcases, or driving or traveling by motor vehicle.
