The Quality & Patient Safety Manager provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well being.

Duties

VA Southern Oregon Rehabilitation Center and Clinics (VA SORCC) is a Level 3 interdisciplinary teaching facility with service lines serving outpatient medical/surgical patients, with 2 community-based outpatient clinics (CBOCs). The Quality & Patient Safety Program Manager (QSPM) is assigned to the Quality Management (QM) department. Duties and Responsibilities include but are not limited to the following: Work closely with the Patient Safety Program Manager (PSPM), Director of QSV, and Deputy Chief of QSV to assess plan, implement, coordinate, monitor and evaluate the quality and safety of the delivery of care for the organization. Assumes a leadership role in collaboration with the QM management and PSPM to facilitate regulatory compliance for internal and external oversight bodies (e.g. The Joint Commission (TJC), Commission on Accreditation of Rehabilitation Facilities (CARF), VHA Directives and Handbooks, etc.), Occupational Safety and Health Administration, Office of the Medical Inspector, Office of the Inspector General, and VHA performance measures. Assists the facility in meeting VHA performance measures, supporting performance improvement work, and infusing patient safety throughout the organization by supporting and guiding the work of multiple committees. Work with the QM Management to lead patient safety initiatives and acts as a subject matter expert in data analysis and data display. Support the Patient Safety Program during the sustainment of the new VA Electronic Health Records (EHR). Work independently and with subject matter experts to address safety issues pertaining to VA EHR. Be the surrogate of the PSPM in his or her absence. Consults and acts as a resource to leadership and staff in Patient Safety principles. Review all patient safety event reports to ensure services have systems in place to provide safe, quality care. The QPSPM, in collaboration with the PSPM, will ensure a minimum of eight patient safety analyses are completed annually. Patient Safety Analyses include the completion of root cause analyses (RCA), proactive risk assessments (PRA), and Patient Safety Assessment Tool (PSAT) evaluations. Participate in the External Peer Review Program (EPRP) as assigned which includes data abstraction and reliable reporting of the data. The QPSPM may be assigned responsibility for completing clinical quality reviews, clinical data analysis, and/or identifying opportunities for improvement. This may include providing clinical and administrative recommendations relative to all Strategic Analytic Improvement Learning (SAIL) outcomes, EPRP metrics and driving improvement initiatives within the Medical Center as assigned by QM management. In addition to supporting the Patient Safety Program, the QPSPM supports all Quality Management work which may include assisting with accreditation, system redesign, infection prevention, risk management, and other duties as assigned by QM management. Advocates for, leads, and manages organizational change to improve the safety and quality of patient care. Competently execute position responsibilities in a manner that demonstrates substantial, continuous, and I- CARE values-based leadership, responsibility, and accountability through support of the Patient Program and other Quality Management responsibilities, as assigned. Collaborates with internal and external stakeholders to promote ongoing development of a just culture and "culture of safety" within VA SORCC. Ensures readiness for accreditation and inspection-related administrative processes and applications for the PSP. Meets goals through efficient, effective utilization and management of organizational and personal resources. The QPSPM also works closely with the PSPM to investigate, evaluate and monitor for adverse and close call patient safety events to identify underlying causes and implement changes needed to reduce the likelihood of recurrence in accordance with High Reliability Processes. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Light lifting, under 15 pounds; Use of Fingers; Both hands required; Walking (2-6 hours); Standing (2-6 hours); Climbing, legs only (.5 hours); Ability for rapid mental and muscular coordination simultaneously; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (aid may be permitted); Sitting for 2-6 hours; Typing for 2-4 hours. Environmental Factors: Outside and inside; Excessive heat; Excessive cold; Slippery or uneven walking surfaces; Working closely with others; Working alone.
