The incumbent is assigned to the Primary Care Service Line as the Chief, Primary Care. The Chief, Primary Care oversees all primary care health services at the VA Southern Oregon Rehabilitation Center and Clinics, including Grants Pass CBOC and Klamath Falls CBOC. This position is 90% Administrative and 10% Clinical.
Duties
Located in the Rogue Valley, White City allows easy access to Metro-area of Medford while remaining engrossed with nature. White City is an unincorporated community in Jackson County, Oregon, in the Rogue Valley. It is about nine miles north of Medford and 34 miles north of the California border. Enjoy the scenic drives to either Crater Lake, Oregon's only National Park, and to the mountain resort Lake of the Woods. Take a family-friendly hike to a waterfall or ride the only section of the Pacific Crest Trail that's open to mountain bikers.
Duties include, but not limited to:
Provides direct patient care through assessment, ordering, monitoring, and follow up where needed by the facility.
Responsible for the initiation of appropriate health care consults and referrals for all direct-care patients as applicable.
Provides consultation to health care providers about assessment findings and treatment plan for all direct-care patients to ensure that care is coordinated, collaborative, and integrated within the Medical Center.
Develops, implements, and documents within the clinical treatment plan for all direct-care patients as indicated and appropriate for the specific job setting and duties of the provider.
Complies with all Service Line documentation requirements, including standards for timely completion of Electronic Medical Health Record (EMHR) patient contact notes, use of required documentation templates, clinical reminders, treatment plans, and risk assessment protocols.
Adheres to all Facility and Service Line policies and practices for delivering direct patient care, including but not limited to direct risk assessment and risk mitigation practices for potentially dangerous behavior (suicide and violence), timeliness of care and access standards, professional conduct requirements, and EMHR documentation conventions.
The Chief, Primary Care has primary responsibility for all aspects of administration both within Primary Care and in the relationship of Primary Care to the VA facility at large and its component service lines.
Develops an overarching strategic vision and plan for the Primary Care Service Line, disseminates the plan to facility staff via respective Clinical Operations Managers (COM), and ensures progress toward the strategic goals.
Exercises supervisory, program management, and administrative responsibilities that are complex in scope and that constitute a significant portion of the facility.
Maintains interdepartmental relations with other services to accomplish facility goals.
Provides leadership with objective, independent assessments and recommendations for policy, operational, and administrative issues and initiatives requiring decision and action.
Advises leadership on implications, key issues, and relationships to interest groups (both internal and external) and recommends courses of action.
Coordinates and negotiates resolutions to complex problems involving the Primary Care Service Line or facility.
Scope of service includes three locations, each with front-line clinical supervisors and administrative staff.
Assesses and collaborates with COMs to reorganize services as new needs and opportunities arise.
The direct administrative and professional supervision of staff (approx. 25 staff)
Service line administrative and professional supervision and accountability all PCSL staff reporting to Clinical Operations Managers or supervisors (above).
Oversees, in collaboration with respective COMs,
The development of policies and procedures, performance standards, position descriptions, and functional statements.
The monitoring of staff clinical performance, and completion of staff performance appraisals. Oversees, in collaboration with respective COMs, other clinical and administrative responsibilities to ensure that the mission of the service and the facility has been satisfied. May set training objectives for staff and delegate responsibilities to subordinate sections.
The management of Human Resource aspects of service line operations, including hiring and disciplinary action, Labor/Management relations, performance appraisals, and professional boarding procedures for various disciplines.

Qualifications
Advanced Practice Nurse (Nurse Practitioner) Basic Requirements
English Language Proficiency. In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or
The Commission on Collegiate Nursing Education (CCNE).
NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment.
License and Certification:
License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and an APN (NP) assignment that are also performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.

The following Scope, Education and Dimension criteria must be met to qualify for Nurse IV:
Scope: Executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines and influence organizational mission and health care.
Education: Master's degree in Nursing (MSN) and 4 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR Master's degree in a *related field with a BSN and 4 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR a Doctoral degree in nursing and 3 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR a Doctoral degree in a *related field with a BSN and 3 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. *Note: Foreign education programs/degrees are not creditable as related degrees.
