This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Performs as a Staff Pharmacist with clinical pharmacy duties with full professional authority, knowledge, and judgment, operating as a generalist with advanced level of responsibility and experience in all aspects of the total Pharmacy operation. Will be active in such areas as Outpatient Pharmacy, Inpatient Pharmacy, Automated Drug Distribution Systems, Long Term Care and Home-Based Primary Care reviews, consultation, procurement, stock control, prepackaging and storage. The incumbent is assigned special reports, projects, and problem matters, requiring an overall or specialized knowledge of the trends, problems, and other matters affecting the total Pharmacy operation. Duties include may not limited to: Accurately interprets medication orders and dispenses medications and/or medical supplies accordingly. Makes appropriate written documentation as required by VA Manual. Reviews medication profiles prior to dispensing medications, Consults with proper medical staff when necessary. Assists in maintaining records of drug reactions, allergies, or other information pertinent to the patient's medication profile. Dispenses controlled substances and is accountable for accuracy of the work. Must have knowledge of the procurement, storage, security and dispensing of requirements as directed by Federal, State, and VA regulations. Quantities of Schedule II and Schedule III substances will be double checked prior to dispensing. Verifies by signature the identification list of medications brought to the facility by patients being admitted. Responsible for the inspection of drugs and biologicals in specific assigned areas. Make inspections monthly and verifies by documented signature. Responsible for the inspection and stock control of drugs and supplies on the emergency carts. Make inspections monthly or as necessary after notification to Pharmacy that a cart has been used. Assumes pharmacist-in-charge assignments and performs the duties of a supervisory position in addition to staff assignment. Reviews patient drug regimens for appropriate indication, dose, drug allergies, and duplication of medications and drug interactions. Monitors the process and outcome of patient care relating to drug therapy. Participates with other members of the primary care team to provide appropriate care for the patient by monitoring appropriate laboratory results and completing information in the patient's medical record. Provides complete patient education/consultation to patient and family members when appropriate. Has Pharmacy Scope of Practice for limited medication refill extensions excluding controlled substances, warfarin and chemo therapeutic agents with stipulations as noted in approved Scope of Practice. Prepares IV admixture solutions according to Doctor's orders. Must have knowledge of physical and chemical incompatibilities and of the stability of IV admixtures under various storage conditions. IV admixtures will be prepared under a Laminar Flow Hood using aseptic techniques to minimize the possibility of contamination. Serves as an independent practitioner in the primary care setting requiring the highest level of pharmacist clinical privileges with minimal supervision. Principal responsibilities include pharmacotherapy management, provision of drug information services, and the development and supervision of pharmacy programs in the primary care firm. Serves as an independent practitioner with maximal prescribing privileges and autonomy under an approved scope of practice. Requires minimal supervision by manager of clinical pharmacy services. Directly assists clinicians in rational drug selection by providing written and/or verbal consultation. Actively participates as a member of a primary care team to assist in monitoring patient's therapy as directly related to pharmaceutical care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS13 Experience. In addition to the GS-12 requirements, must have one year of experience equivalent to the next lower grade level. Demonstrated KSAs. The KSAs shown below apply to all assignments at this grade level. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. (KSAs must be clearly reflected in resume under description of duties in order to be found qualified for this position/grade level). Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Preferred Experience: One Year Clinical Externship preferred but not required References: VA Handbook 5005/55 Part II Appendix G15 The full performance level of this vacancy is GS-13. Physical Requirements: The work involves prolonged periods of standing with some walking of generally short distances. There may be occasional lifting of weights up to 30 lbs. There may be mental fatigue due to repetitive screening and interpretation of medication orders.
