This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Total-Rewards-of-an-Allied-Health-VA-Career-Brochure.pdf The primary responsibility of the Women's Health, Clinical Pharmacy Specialist (Women's Health CPS) is the evaluation of medication therapy through direct patient care assessment. Their direct patient care activities may include but are not limited to individual visits, group visits, telephone visits, video conference visits, chart review, and use of secure messaging to serve women veterans across the Alexandria VA Health Care System (AVAHCS) and receiving care at AVAHCS Community Based Outpatient Clinics (CBOCs) that are not meeting women's health equity indicators. Through clinical assessment, the Women's Health CPS relate patient responses to medication therapy, communicate and document those findings, make recommendations to appropriate individuals and implement and monitor pharmacotherapeutic care plans for female gender specific conditions including maternity Duties include but are not limited to: Responsible for outpatient ambulatory care pharmacy activities involving highly innovative methods of health care delivery in clinic. Includes direct patient care services through individual visits, group visits, telephone visits, video conference visits, chart review, and use of secure messaging. Participates in a patient-focused, team-based multi-disciplinary model of patient centered care to promote positive patient outcomes through direct and indirect interaction with patients and providers. Serves as a mid-level provider by directing therapy; ordering lab tests; performing physical assessments; and initiating, modifying and discontinuing medications as defined within the Clinical Pharmacy Specialist's scope of practice. The incumbent will be evaluated through Clinical Pertinence Review for scope of practice. After SOP is authorized the incumbent will enter orders for medications, laboratory tests, devices and consults granted under Scope of Practice. Orders and consults will be accurate and appropriately documented. Physicians or other appropriate health care professionals are contacted when necessary. Patient care will be appropriately and accurately documented. Conducts medication therapy management clinics and group forums (e.g. diabetes, smoking cessation, gout, pain, dyslipidemia, etc.). Provides drug information, observations on patient response to therapy, and develops appropriate treatment plans regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration choice of therapy, safety, efficacy, and pharmacoeconomic. Evaluates appropriateness of pharmacologic therapy at clinic visits and as the level of care changes. Develops appropriate therapeutic monitoring plans and documents the activities in the medical record via electronic progress notes. Provides medication reconciliation and patient medication counseling verifying and documenting patient understanding of the medication, proper medication use, expected outcomes, how to obtain refills of medication and how the current prescription will be processed. vii. Reports actual and/or potential medication errors or adverse events and participate in quality improvement initiatives to promote patient safety. Actively participates in the VA Adverse Drug Reaction (VA ADR) reporting program. Work Schedule: Monday - Friday; 8:00am - 4:30pm, subject to change based on the needs of the facility. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL IBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS13: Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Demonstrated KSAs. The KSAs shown below apply to all assignments at this grade level. a . Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b . Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Physical Requirements: Moderate lifting 15-44pounds, Light lifting under 15 pounds, Moderate and light carrying. Pushing, 2 hours. Reaching above shoulder, use of fingers, both hands required.
