Incumbent is an advanced practice social worker whose duties and responsibilities relate to the facility's provision of services for Veterans who have experienced Military Sexual Trauma (MST) and direct service provision (case management and treatment) for these Veterans. This position involves coordination of services at the John J. Pershing VAMC including associated CBOCs and local community agencies.

Duties

Major Duties: Coordinator Functions: 50% 1. Responsible for supporting the mission, policies, and procedures of VA, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. 2. Subject matter expert on VHA MST related directives, handbooks, memos and all other program-related guidance. Tracks facility MST metrics and ensures processes are enacted to meet those metrics. 3. May serve on committees, work groups, and task forces at the facility, VISN, and/or VA Central Office levels. 4. Must maintain a level of productivity and quality consistent with facility and departmental standards and the complexity of the assignment. 5. Participates in interdisciplinary team meetings, appropriate facility meetings, and Social Work meetings. Shares knowledge and experiences gained from own clinical practice and education relevant to the field of Social Work. 6. Facilitates liaisons with other health care programs/services in order to effectively coordinate services to Veterans who have experienced MST. 7. Establishing a network of community resources and agencies that can be used as referral sources to enhance MST related services to veterans in need. 8. Establishing connections with MH service to help expand and enhance existing MST services. 9. Assisting the facility in accurately monitoring MST related service needs and helping to monitor and coordinate MST-related services in John J. The incumbent will conduct psychosocial and diagnostic assessments to determine needs of Veterans who have experienced MST and their family members. 2. The incumbent is an active participant in the collaborative treatment planning process with other disciplines. 3. The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. 4. The incumbent will provide recovery-oriented individual, family and group psychotherapy as appropriate. 5. The incumbent will provide consultation/education to Veterans, their families, and VA staff on community resources, advance directives and VA benefits specific to MST. 6. The incumbent will act as a consultant to other team/staff members regarding best practices (e.g., evidence-based practice) in treating Veterans who have experienced MST. 7. The incumbent will act as liaison between VA and community, to include education on various aspects of MST, representation at community events, and other public service, as needed. 8. The incumbent will provide case management to Veterans and families, as needed, throughout the continuum of care. 9. The incumbent will provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group psychotherapy, discharge planning, case management and other services as appropriate. Evidence based practices will be incorporated into services, as applicable (e.g., Cognitive Processing Therapy, Prolonged Exposure, CBT, etc.). 10. The incumbent will address Advance Directives and Organ Donation in accordance with the standards of their work area. 11. The incumbent will provide wellness/prevention education and facilitate Patient/family support groups as appropriate. 12. The incumbent will as appropriate, supervise the practice of social work students, or a less experienced social worker. 13. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for Veterans. 14. The incumbent will participate in committee assignments, as deemed necessary by the supervisor. 15. The incumbent will maintain data and statistical compilations to comply with Joint Commission, OIG etc., VA and VA Health Care policy and procedures. 16. The incumbent will document social work interventions and activities in patient's clinical record utilizing the electronic medical record and ensure appropriate coordination of care when transferring of patient care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work. Verification of the degree can be made by going to the CSWE website (www.cswe.org) to verify if that social work degree meets the accreditation standards for a Master of Social Work. Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree levels. English Language Proficiency: Candidates must be proficient in spoken and written English as required by 38 U.S.C. 7403(f). Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.] Preferred Experience: Experience in recovery oriented care and outreach References: VA Handbook 5005, Part II, Appendix G39 dated September 10, 2019.
